基于土壤学说探讨精子DNA碎片率升高论治思路及经验

摘要 精子DNA碎片率升高是男性不育症的重要病因,是导致反复性流产、造成妊娠和辅助生殖技术不良结局的重要因素。现代医学对精子DNA碎片率升高尚无特异性治疗方法,多根据临床经验进行综合治疗,疗效尚不确切。李曰庆和李海松教授根据古籍记载和文献报道,结合临床经验提出睾丸生精的土壤学说,认为肾虚血瘀、兼夹瘀热等是睾丸生精障碍、精子质量差的核心病机,并形象地将补肾称为“施肥”,活血称为“浇水”,临症治疗该疾病常采用补肾填精、活血清热之法,取得较好临床疗效。 The increase in sperm DNA fragmentation index is a significant etiology of male infertility and an important factor leading to recurrent miscarriages,as well as adverse outcomes in pregnancy and assisted reproductive technologies.Modern medicine currently lacks specific treatments for increased sperm DNA fragmentation index,and comprehensive treatments based on clinical experience are often employed,though their efficacy remains uncertain.Drawing on insights from ancient texts and literature,combined clinical practice,professors Li Yueqing and Li Haisong developed the soil theory to describe testicular spermalogenesis.Kidney deficiency,blood stasis,and stasis-heat are identified as the central pathological mechanisms driving impaired testicular spermatogenesis and poor sperm quality.They metaphorically equate kidney tonification to"fertilization"and blood circulation promotion to"watering".Clinically,their treatment strategies typically include kidney tonification to nourish essence,blood activation to enhance circulation and heat clearance,resulting in positive therapeutic outcomes.
