摘要
目的 运用根本原因分析法(RCA)结合七何分析法(5W2H)改进“伏九贴”贴敷工作。方法 成立RCA小组,采用时序法收集数据,运用RCA,从人员、材料、制度、环境等方面剖析2019—2021年“伏九贴”贴敷人次逐年下降的近端原因,应用鱼骨图分析并确定根本原因,运用5W2H分析法改进贴敷工作,并从服务人次、患者满意度、团队能力等方面进行效果评价。结果 RCA结合5W2H分析法改进贴敷工作后,服务人次明显增加,就诊患者满意度由81.81%提升至96.36%,团队能力提升,改进效果显著。结论 运用RCA分析法能有效找到“伏九贴”贴敷人次逐年下降的根本原因,结合5W2H分析法,可准确界定、清晰表述问题,并制定改善方案,提高“伏九贴”贴敷工作效率。
Objective To improve the application of Fujiu Patch by combining root cause analysis(RCA)with the 5W2H(what,why,who,when,where,how,and how much)analysis method.Methods An RCA team was established,and event-related data were collected using the chronological method.RCA was used to analyze the proximate causes of the annually decline in the number of Fujiu Patch applications from 2019 to 2021 from the perspectives of people,materials,systems,and environment.The root cause was determined and analyzed using the fishbone diagram analysis.And then the 5W2H analysis method was utilized to improve the application work.The effects were evaluated from the perspectives of service visits,patient satisfaction and team capabilities.Results After the application work was improved by RCA combined with 5W2H analysis method,the number of service visits increased significantly,the patient satisfaction increased from 81.81%to 96.36%,the team ability was improved,and the improvement effects were remarkable.Conclusion RCA can effectively find the root causes of the decline in the number of Fujiu Patch applications year by year.After combining it with the 5W2H analysis method,the problems can be accurately defined and clearly stated,thus the improvement action plan can be formulated to improve the work efficiency.
