中性粒细胞/淋巴细胞比值对老年非瓣膜性心房颤动患者心血管死亡及全因死亡的预测价值

摘要 目的 探讨中性粒细胞/淋巴细胞比值(NLR)与老年非瓣膜性心房颤动(NVAF)患者全因死亡及心血管死亡的相关性。方法 本研究采用回顾性队列研究,选取2022年1—12月就诊于新疆维吾尔自治区人民医院的老年NVAF患者为研究对象,进行生存情况的随访。采用Cox比例风险回归模型和Kaplan-Meier生存曲线评估NLR与老年NVAF患者死亡率的关系。采用限制性立方样条曲线评估NLR与死亡风险的影响,ROC曲线分析NLR的预测能力。结果 共纳入484例患者,高NLR水平组患者的全因死亡及心血管死亡风险显著增加。Cox回归分析显示NLR是老年NVAF患者全因死亡的独立预测因素。NLR对老年NVAF患者死亡风险具有较好的预测能力。结论 NLR水平升高与老年NVAF患者死亡风险具有显著相关,有助于识别临床预后较差的老年NVAF患者。 Objective To investigate the correlation between neutrophil to lymphocyte ratio(NLR) and all-cause mortality and cardiovascular mortality in elderly patients with non-valvular atrial fibrillation(NVAF).Methods This study adopted a retrospective cohort study,selecting elderly NVAF patients who visited People's Hospital of Xinjiang Uygur Autonomous Region from January 2022 to December 2022 as the research subjects,and conducting follow-up on their survival status.Using Cox proportional hazards regression model and Kaplan-Meier survival curve to evaluate the relationship between NLR and mortality rates in elderly NVAF patients.Using restricted cubic spline curves to evaluate the impact of NLR on mortality risk,and analyzing the predictive ability of NLR using ROC curve.Results A total of 484 patients were included.The risk of all-cause mortality and cardiovascular mortality is significantly increased in patients with high NLR levels.Cox regression analysis showed that NLR is an independent predictor of all-cause mortality in elderly NVAF patients.NLR has a good predictive ability for the mortality risk of elderly NVAF patients.Conclusion Elevated NLR levels are significantly correlated with the risk of mortality in elderly patients with NVAF,which helps identify elderly patients with poor clinical prognosis.
