摘要
目的研究输尿管镜钬激光碎石术对老年输尿管结石患者的治疗效果,并分析其对患者肾功能的影响,以期为临床治疗提供参考。方法本研究回顾性选取2015年1月—2023年12月江苏省泰州市第四人民医院收治的72例输尿管结石患者的临床资料,根据治疗方式差异分为对照组(24例,接受输尿管镜气压弹道碎石术治疗)、探究组(48例,采用输尿管镜钬激光碎石术治疗)。对比两组手术相关指标、炎性因子水平、肾功能、并发症发生率。结果相较于探究组,对照组术中失血量高,手术操作时间、下床时间及住院时间均较长,差异均有统计学意义(P均<0.05)。治疗后,探究组肾功能水平优于对照组,炎性因子水平低于对照组,差异均有统计学意义(P均<0.05)。探究组并发症总发生率为4.17%(2/48),低于对照组的25.00%(6/24),差异有统计学意义(χ^(2)=5.080,P<0.05)。结论输尿管镜钬激光碎石术可有效治疗老年输尿管结石,不仅能减少术中失血量和手术时间,同时,该手术方法还有助于患者迅速恢复,并减少并发症的出现,改善炎性因子水平,对肾功能的保护效果更佳。
Objective To study the therapeutic effect of ureteroscopic holmium laser lithotripsy on elderly patients with ureteral calculi,and to analyze its effect on renal function,so as to provide reference for clinical treatment.Methods The clinical data of 72 patients with ureteral calculi admitted to the Fourth People's Hospital of Taizhou City,Jiangsu Province from January 2015 to December 2023 were retrospectively selected.According to the differences in treatment methods,the patients were divided into the control group(24 cases,treated with ureteroscopic pneumatic lithotripsy)and the exploration group(48 cases,treated with ureteroscopic holmium laser lithotripsy).The surgicalrelated indexes,levels of inflammatory factors,renal function,and the incidence of complications were compared between the two groups.Results Compared with the exploration group,the control group had a higher intraoperative blood loss,and the operation time,time to get out of bed,and hospital stay were all longer,and the differences were statistically significant(all P<0.05).After treatment,the renal function level of the exploration group was better than that of the control group,and the levels of inflammatory factors were lower than those of the control group,and the differences were statistically significant(all P<0.05).After treatment,the levels of inflammatory factors in the two groups were lower than those before treatment,and the levels in the inquiry group were lower than those in the control group,the differences were statistically significant(all P<0.05).The total incidence of complications in the inquiry group 4.17%(2/48)was lower than that in the control group 25.00%(6/24),and the difference was statistically significant(χ^(2)=5.080,P<0.05).Conclusion Ureteroscopic holmium laser lithotripsy can effectively treat ureteral calculi in the elderly.It can not only effectively reduce intraoperative blood loss and operation time,but also help patients recover quickly,reduce the occurrence of complications,improve the level of inflammatory factors,and better protect renal function.
