摘要
目的:探讨前列腺蓝激光汽化术治疗良性前列腺增生的安全性和有效性,以及评估该技术对不同年资泌尿外科医师的学习曲线。方法:回顾性分析2022年10月—2024年10月在上海交通大学医学院附属新华医院行前列腺蓝激光汽化术84例患者的临床资料。在有完整随访数据的32例患者中,分析术前和术后第1、6、12个月的国际前列腺症状评分、生活质量评分、尿流率、残余尿、手术并发症,以及尿控和性功能的情况,判断该术式的有效性和安全性。并通过手术效率与学习曲线拟合,研究对比新手与资深医生对该技术的适应情况。结果:蓝激光在术后1个月能显著改善前列腺梗阻症状,效果维持1年以上。同时在尿控及性功能保护方面表现优越,手术后未发生尿失禁等严重并发症。对于掌握前列腺增生手术技巧的医生,前列腺蓝激光汽化术的学习曲线为9~11例,初学者17例操作后也能达到成熟期。结论:前列腺蓝激光汽化术具有安全高效、学习曲线短的优势,有望在前列腺增生治疗中得以推广。
Objective:To explore the safety and effectiveness of 450 nm semiconductor blue laser in the treatment of benign prostatic hyperplasia(BPH),and analyze the learning curve of this technology.Methods:This study retrospectively analyzed the clinical data of 84 patients underwent prostate blue laser vaporization at Xinhua Hospital affiliated with Shanghai Jiao Tong University School of Medicine from October 2022 to October 2024.Among the 32 patients with complete follow-up data,we analyzed the International Prostate Symptom Score,quality of life,urine flow rates,residual volume,and surgical complications(continence and sexual function)before surgery and at 1,6,and 12 months postoperatively to assess the effectiveness and safety of the procedure.The learning curves of novice and experienced doctors were compared by fitting the surgical efficiency.Results:The blue laser significantly improved prostatic obstruction symptoms one month after surgery,with effects lasting for more than one year.It also demonstrated superior performance in preserving urinary continence and sexual function,without serious complications occurring after surgery.For surgeons who have mastered the techniques of BPH surgery,the learning curve for the blue laser is 9-11 cases,and beginners can reach a proficient level after 17 operations.Conclusion:Through this study,we found that the 450 nm semiconductor blue laser has the advantages of high efficiency,safety,and a short learning curve,so it is expected to be widely promoted in the treatment of BPH.
