摘要 目的 :探讨二维彩色多普勒超声显像对下肢深静脉血栓形成的诊断价值。方法 :对临床疑诊下肢深静脉血栓形成患者 90例 ,1 1 8条病肢 ,应用二维彩色多普勒超声检测下肢深、浅静脉管壁、管径及周围结构 ,血栓部位、大小、范围 ,彩色血流变化 ,检测静脉瓣功能。结果 :二维彩色多普勒超声共检出阳性患者 41例 ,53条病肢。中心型 1 2条 (2 2 .6 % ) ,周围型2 5条 (47.2 % ) ,混合型 1 6条 (30 .2 % )。病肢左右侧之比 3 .1 :1 (P <0 .0 0 1 )。结论 :诊断下肢深静脉血栓形成二维彩色多普勒超声具有特异性表现 ,在其病因及危险因素判定方面可以起到筛选作用 ,可作为下肢不明原因肿胀的首选方法。同时在观察病程演变及对静脉瓣功能影响等方面 。 Objective:To explore the value of two dimensional ultrasonography(2D) and color Doppler flow imaging (CDFI) in detection of deep venous thrombosis of the lower limbs.Methods:Ninety patients,118 limbs clinically suspected as deep venous thrombosis were examined by 2D and CDFI.The resulted causes, the symptoms, the regression, the size, the degree of infarction, the blood flow and the function of the valves etc were detected and analysed.Results:Fourty one cases were diagnosed by 2D CDFI and 53 limbs were concerned.The central locality of thrombosis in 12 limbs(22.6%), peripheral 25 limbs(47.2%), both central and peripheral 16 limbs(30.2%) were identified.The left to right occurrence rate were 3.1:1 (P<0.001).Conclusion:There are characteristic ultrasonic and color Doppler flow imaging in each stage of thrombosis.2D CDFI becomes the first technique used in the primary detection of unidentified limb swelling .It can be used to observe evolvement of deep venous thrombosis dynamically and the affection to the valve.It is prior to other medical imaging technology.
