孤立性肺腺癌血流模式定量CT参数相互关系的初步研究 被引量:9

Preliminary study of the relationships among the quantifiable parameters derived with dynamic CT about blood flow pattern of solitary bronchogenic adenocarcinoma

摘要 目的 :探讨孤立性肺腺癌血流模式定量CT参数相互关系 ,以便设计简便的扫描程序研究肺腺癌血管生成。方法 :2 9例孤立性肺腺癌 (直径≤ 4cm)患者行多层螺旋CT(MSCT)动态增强 (以 4ml/s的流率注入对比剂 90ml)。记录孤立性肺腺癌的定量参数 (强化值、肺腺癌 主动脉强化值比、灌注量和平均通过时间 )。用线性回归分析评价主动脉强化值与孤立性肺结节的参数及各参数间的关系 ,并经逐步回归建立参数间的回归方程。结果 :肺腺癌强化值 (PHBA)为(36 .86± 1 3 .0 6)HU ,与动脉强化值 (PHA)呈正相关 (r =0 .586 ,P =0 .0 0 1 ) ,肺腺癌与大动脉强化值比 (BA to Aratio)为1 5 .58± 4 .52及灌注值 (PBA)为 (32 .2 7± 1 1 .75)ml/ (min·1 0 0g)与动脉强化值无显著相关 (r=0 .0 0 0 1 ,P >0 .0 5 ;r=0 .0 86 ,P >0 .0 5) ,肺腺癌灌注值与强化值及肺腺癌与大动脉强化值比呈正相关 (r =0 .437,P <0 .0 5 ;r=0 .468,P <0 .0 5) ,肺腺癌强化值与肺腺癌与大动脉强化值比呈正相关 (r =0 .80 1 ,P <0 .0 0 1 ) ,平均通过时间 (1 7.2 7± 4 .60 )s,PBA=1 3 .2 93 +1 .2 1 8·BA to Aratio ,BA to Aratio=5 .371 +0 .2 77·PHBA。结论 :可用二元一次方程式表示肺腺癌灌注值、强化值及肺腺癌与大动脉强化值比的线性关系 ,? Objective:To evaluate the relationships among the quantifiable parameters derived with dynamic CT about blood flow pattern of solitary bronchogenic adenocarcinoma (SBA) in order to design a simpler scanning procedure of investigation of bronchogenic adenocarcinoma angiogenesis.Methods:29 patients with SBA(diameter ≤4cm)underwent dynamic contrast enhanced (90ml,4ml/s) multi slice CT.Peak heights of solitary bronchogenic adenocarcinoma (PH BA )and the aorta (PH A),perfusion (PBA)and mean transit time(MTT) were measured.Ratio of peak height of the bronchogenic adenocarcinoma to that of the aorta(BA to A ratio) was calculated.The relationships between peak height of the aorta (PH A) and parameters of the SBA (PH BA ,BA to A ratio,P BA ,and MTT) and those among parameters of the SBA were assessed by means of linear regression analysis.Regression equation among parameters of the SBA was obtained by means of stepwise regression.Results:The correlation between the SBA peak height (PH BA ,36.86±13.06HU) and the aortic peak height (PH A)was significant( r =0.586, P =0.001).No significant correlation was found between the BA to A peak height ratio(15.58±4.52) and the aortic peak height( r =0.0001, P >0.05)as it was between the SBA perfusion (P BA ,32.27± 11.75ml/min/100g) and the aortic peak height(r =0.086, P >0.05).The SBA perfusion correlated with the PH BA and the BA to A peak height ratio( r =0.437, P <0.05; r =0.468, P <0.05).The PH BA correlated positively with the BA to A peak height ratio( r =0.801, P <0.001).Mean transit time was 17.27±4.60s. P BA =13.293+1.218·BA to A ratio.BA to A ratio=5.371+0.277·PH BA .Conclusion:The linear relationship between the SBA perfusion and BA to A ratio and that between BA to A ratio and PH BA can be expressed by equation.It is possible to design a simpler scanning procedure of investigation of bronchogenic adenocarcinoma angiogenesis.
