大鼠肝移植后肝切除-肝再生功能的探讨 被引量:2

Investigation on liver regeneration after rat liver transplantation

摘要 目的 探讨肝移植后肝细胞的再生功能。方法 从肝左静脉根部到肝脏面的肝右叶和肝中叶交叉点进行肝切除。肝切除后定时屠杀,标本行苏木素-伊红(HE)、增殖细胞核抗原(PC-NA)染色检查。血液行动脉血酮体比(AKBR)测定、血清白蛋白测定。结果 术后第3天对照组的再生肝对残肝重量比为(2.19±0.18)倍,3、6、12个月实验组分别为(1.66±1.11、1.66±0.17、1.17±0.31)倍,与对照组相比差异有显著性(P<0.05)。对照组术后 1 d的 PCNA染色阳性细胞总数达最高值(459.2±21.5)个,7 d为(35.7±7.2)个。3、6、12个月实验组术后7 d分别为(356.2±40.8)个、(341.2±17.6)个、(359.7±23.0)个,与对照组相比差异有显著性(P<0.001)。实验组第3、7天的血清白蛋白值比对照组降低。对照组术后 1、3 d的 AKBR值分别是 0.56±0.26和0.79±0.12,全部实验组的 NBR值术后显低值,3、6、12个月实验组术后 1 d的 AKBR值分别为0.33±0.12、0.31±0.12、0.30±0.05,3 d分别为 0.29±0.09、0.28±0.09、0.26±0.15,与对照组相比差异均有非常显著性(P<0.01)。结论 移植肝脏肝切除后的再生功能,比正常肝组织缓慢。肝移植后肝脏的预备能减弱,再生延缓。 Objective To investigate liver regeneration following rat liver transplantation. Meth- ods Orthotopic liver transplantation models of LEW-LEW rats were established and divided into 3 groups by survival time (experimental groups of 3, 6, 12 months). Normal LEW rats served as control group. Liver from root of left hepatic veins to borderline of right lobe and medial lobe on visceral surface was resected. Rats were killed at 1st, 3rd, 7th and 14th day after resection, blood was collected from big artery in abdomen, and residual liver was weighed, fixed with 10% formalin, stained with H-E and prolif- erating cell nuclear antigen (PCNA). Arterial ketonl body ratio (AKBR) was measured. Results The weight of regenerative liver was 1. 37 times of residual liver at 1st day after liver resection in control group, 2. 19 times at 3rd day, 2. 88 times at 7th day and 2. 94 times at 14th day. While the weight of re- generative liver was 1. 66±1. 11 times, 1. 66±0. 17 times and 1. 17±0. 31 times of residual liver at 3rd day after liver resection in the experimental groups of 3, 6, 12 months. There was significant difference between the control group and experimental groups (P < 0. 05). The amount of PCNA stained positive cells reached peak of 459. 2±21. 5 at 1st day after operation in control group, then decreased gradually, 35. 7±7. 2 at 7th day, while in the experimental groups of 3, 6, 12 months it was 356. 2±40. 8, 341. 2± 17.6 and 359. 7±23. 0 respectively at 7th day, which was significantly different from that in control group (P< 0. 001). The value of AKBR was 0. 56±0. 26 at 1st day and reached peak of 0. 79±0. 12 (1. 37 times of preoperation) at 3rd day after operation in control group, and it was lower than that pre- operation at 14th day. The value of AKBR in the experimental groups of 3, 6, 12 months was 0. 33± 0. 12, 0. 31±0. 12, 0. 30±0. 05 respectively at 1st day, and 0. 29±0. 09, 0. 28±0. 09, 0. 26±0. 15 re- spectively at 7th day, which was significantly different from that in the control group (P < 0. 01). Con- clusion The regenerative function of transplanted liver after resection was slower than normal liver in proliferation and differentiation and the reserve of liver weakened and regeneration retarded.
