摘要
利用培养的人表皮细胞片同种移植于烧伤创面,最长随访至半年未见明显排异。为究其原因,对表皮内与排异密切相关的DR抗原的Langerhans细胞在培养过程中的变化进行了观察。证明新生的表皮细胞片内不显示有Langerhans细胞,在体外和异体淋巴细胞混合培养也不具有刺激转化的现象。提示培养的表皮细胞片同种移植,有可能延长存活期。
Clinical application of cultured epidermal cells has been documented.Human skin explants (<1 mm3) were cultured with DMEM (containing 10% neonatal bovine serum) in petridish at 37℃.in 5%CO2 incubator.The epidermal cells began to proliferate in 1-3 days and became confluent as a sheet,formed by 4-5 layers of cells,in 2-3 weeks.When used as a graft,it was deteched as a whole about 5 cm in diameter from the dish.Fifty two sheets of cultured allogenic epidermal cell were transplanted onto the wounds of 22 cases of burn patients.All of the 11 sheets grafted on donor areas were taken well.Eleven of 13 sheets grafted on escharectomized wound also showed good take,with a survival rate of 84.6%.On the tangentially excised wounds,5/10 sheets were taken,a survival rate of 50%.Eighteen sheets were covered on granulation wounds,only 3 of them,16.7%,were taken.Three patients were followed up to 6 months after grafting.The round contour of the grafted sheets was still discernible and it was smoother with less pigmentation than that of autogenic split skin grafts.Since Langerhans cells(LC),the sole cells bearing DR antigen in skin,were considered as playing an important role in allogeneic skin graft rejection,the changes in LC during the course of epidermal cell culture was observed.Three days after incubation,LC in the epithelial explant showed marked degeneration and decreased in number.They almost disappeared entirely at the end of 2 weeks.Changes in mixed epidermal cell and allogeneic lymphocyte reaction were closely parallel with the histomorpho-logical changes,declination of the stimulating ability of EC was observed on the 3rd post-inoculation day,and there was nearly no proliferation response as showed by the allogeneic lymphocytes at the end of one week.The new growth of epidermal cells around the original skin explant exhibited no stimulating effects on allogeneic lymphocytes and no LC was found in it.These results suggest that the survival of allogeneie cultured epidermal cell sheet grafting may be prolonged.Probably,it can be served as a long term burn wound covering.
