摘要
目的 :为系统研究肝脏的管道结构 ,建立适于薄层CT扫描和三维重建的肝脏标本管道系统灌注和铸型的方法 .方法 :肝门部保留完好的完整 1 4个肝脏 ,经预处理后 ,分别以含两种不同浓度的显影物质的灌注材料对各管道灌注 .灌注完好的肝脏标本经固定和铸型 ,置于模拟腹腔模具内 ,行CT薄层扫描 ,以最大密度投影法 (MIP)、表面覆盖显示法(SSD)进行肝脏管道结构的三维重建 .比较各管道的显影和三维成像情况 .结果 :肝脏标本预处理和灌注顺利 ,细小管道灌注充分 .肝脏标本CT扫描的层面管道结构清晰 ,在标本扫描的中间部位 ,门腔静脉 肝右 肝中和肝左静脉 ,左 右肝段的门静脉分支清晰可见 .三维重建各管道分支明确 ,立体感强 .结论 :肝脏管道系统灌注和铸型操作简单 ,适于肝脏薄层CT扫描和管道系统三维重建研究 ,是研究肝脏管道结构的较理想的方法 .
AIM: To establish the method of perfusion and casting of hepatic duct system for thin slice CT scan and three dimensional computerized reconstruction for the study of the structure of hepatic duct. METHODS: Fourteen livers with integra porta hepatis were perfused with various filling material containing visualized substance after pretreatment, and then fixed and cast. Hepatic preparations that had been perfused were put into the model of the modeling abdominal cavity, which was then scanned with thin slice. Three dimensional computerized reconstruction of the structure of each hepatic duct was carried out by shaded surface display (SSD) or maximum intensity projection (MIP) according to the CT laminogram, and the results were contrasted and analyzed. RESULTS: Tiny canals were fully perfused and the structure of hepatic duct was clearly showed by CT laminogram. Vena cava and portal vein, right hepatic vena and intermediate vena and left hepatic vena, and right branch of portal vena and left branch of portal vena were all clearly seen. All branches of hepatic duct were clear cut and stereoscopic visions were transparent on the images of three dimensional computerized reconstruction. CONCLUSION: The technique of perfusion and cast of hepatic duct system is simple, and good for scanning liver with thin slice by CT and making three dimensional computerized reconstruction of hepatic duct system. It is a good method to study the hepatic duct system.
