双螺旋CTA诊断脑动脉瘤的价值

摘要 目的 评价CTA诊断脑动脉瘤的价值。方法  5 0例以 2~ 3ml/s从肘静脉注射碘造影剂 4 0~ 10 0ml,延迟 12~ 2 0s行连续容积扫描 ;资源图像输入独立工作站 ;采用多平面重建 (MPR)、最大密度投影 (MIP)、表面遮盖显示(SSD)和仿真血管微镜 (VM )重建 ;与DSA对比 ,并栓塞治疗。结果 ①双螺CT扫描能在短时内完成数据采集 ;②MPR、MIP能准确显示动脉瘤的大小、部位 ,SSD、VM显示空间解剖及内腔效果好 ;③CTA与DSA有高相关性 (r >0 99)。结论 注重容积信息与多重建模式互补选择 。 Objective To evaluate the clinical value of helical CT angiography for cerebral aneurysm. Methods 50 cases were scanned with Hispeed NX/i scanner. A volume of 40~100 ml iodine contrast was injected at rate 2~3 ml/s with a scan delay of 12~20 s. Single continuous volume scan was used with 1~2 mm collimation,pitch 1 2~1 5 and reconstruction interslice distance 0 4~0 8 mm. The data were transferred to an independent workstation AW 4 0. The source images were reconstructed with multiplanar reconstruction(MPR), maximum intensity projection(MIP), shaded surface display(SSD), and virtual microscopy(VM). Results ①Twin helical CTA was able to demonstrate a longer segment of Willis' artery with a thinner slice and a shorter scan time. ②The diameter and location of aneurysm could be accurately shown with MPR and MIP. The SSD and VM was able to provide some information about the morphology of space and inner cavity of aneurysm. ③The sensitivity and accuracy of CTA on the diagnosis of cerebral aneurysm was 98% and 98%, respectively. Conclusion Emphasizing collecting scan volumetric data with moderate 3D reconstruction model, CTA can provide more information in diagnosis of cerebral aneurysm.
