空肠营养管在食管癌三野手术围手术期中的应用及其对营养状态改善的研究 被引量:2

Application of jejunal nutrition tube in perioperative period of three-field surgery for esophageal cancer and its improvement on nutritional status

摘要 目的探讨空肠营养管在食管癌三野手术中的应用,及对术后患者营养状况的改善作用,为临床治疗提供参考。方法采用回顾性横断面研究方法,收集2010年1月至2018年1月在福建省肿瘤医院胸外科完成食管癌手术的568例患者的临床资料,均完成食管癌三野根治术。术中完成空肠造口术,并在术后4周内完全经空肠营养管进行肠内营养317例(管饲组);术中未完成空肠造口术,在术后第7天起开始经口进食251例(口服组)。分析并比较两组患者临床资料;术后第4周复查,检测并比较两组患者白蛋白和前白蛋白指标水平。结果两组手术方式差异无统计学意义(P>0.05)。管饲组平均年龄为(61.79±9.24)岁,与口服组的(60.46±8.98)岁比较,差异无统计学意义(P>0.05);两组患者性别、病理类型、肿瘤位置、分化程度、TNM分期比较,差异均无统计学意义(P>0.05)。术后第4周,管饲组白蛋白和前白蛋白水平均明显高于口服组,差异有统计学意义(P<0.05)。结论术中留置经皮空肠营养管、术后通过空肠营养管进行肠内营养对于食管癌术后患者营养状况的改善具有重要的作用。 Objective To investigate the application of jejunal nutrition tube in the three-field operation of esophageal cancer, and to improve the nutritional status of postoperative patients and provide reference for clinical treatment. Methods A retrospective cross-sectional study was conducted to collect the clinical data of 568 patients who underwent esophageal cancer surgery in the Department of Thoracic Surgery, Fujian Provincial Cancer Hospital from January 2010 to January 2018, and completed the three-field radical surgery for esophageal cancer. Intraoperative jejunostomy was performed, and 317 cases of enteral nutrition were completely administered through the jejunal nutrition tube within 4 weeks after operation(tube feeding group). The jejunostomy was not completed during the operation, and the import began on the 7th day after surgery in 251 cases(oral group). The clinical data of the two groups were analyzed and compared. The levels of albumin and prealbumin were measured and compared in the 4th week after surgery. Results There was no significant difference in the surgical methods between the two groups(P>0.05). The average age of the tube-fed group was(61.79 ±9.24) years old andwas(60.46±8.98) years in the oral group, and the difference was not statistically significant(P>0.05). There were no significant differences in gender, pathological type, tumor location, differentiation degree and TNM staging between the two groups(P>0.05). At the 4th week after operation, the albumin and prealbumin levels in the tube-fed group were significantly higher than those in the oral group(P<0.05). Conclusion Intraoperative percutaneous jejunal nutrition tube and postoperative enteral nutrition through jejunal nutrition tube play an important role in improving the nutritional status of postoperative patients with esophageal cancer.
