摘要
目的探讨直接面视下全程监督化学治疗(DOTS)治疗下肺结核空洞的影像学表现及其变化规律,为临床诊治肺结核提供评价方法。方法收集42例经DOTS治疗的肺结核患者的首诊、2个月末、6个月末、12个个月末四个节点的影像资料,并对总计51个空洞进行动态观察。结果肺结核空洞以单发厚壁多见,多有卫星灶;2个个月末闭合率27.5%,6个月末闭合率74.5%,12个个月末闭合率90.2%。结论肺结核空洞影像特征为多种性质的卫星灶与空洞并存,并可见支气管引流及播散灶;动态观察有助于结核病的鉴别诊断、确定其活动性及评价预后。
Objective To explore the imaging manifestations and changes of pulmonary tuberculosis cavity under directly observed treatment+short course chemotherapy treatment,and to provide an evaluation method for clinical diagnosis and treatment of pulmonary tuberculosis.Methods Imaging data of42 patients with pulmonary tuberculosis treated with DOTS were collected in four time nodes,at first/at the end of February/June and December respectively,and a total of 51 holes in the dynamic observation.Results Tuberculosis cavities were more common in single-walled and thick-walled,and more satellite-shaped stoves;the closing rate at the end of February was 27.5%,the closing rate at the end of June was 74.5%,and the closing rate at the end of December was 90.2%.Conclusion The image of tuberculosis cavity is characterized by multiple types of satellite foci and cavities,and bronchial drainage and disseminated foci are visible.Dynamic observation can help the differential diagnosis of tuberculosis,determine its activity and evaluate prognosis.
