摘要
目的 :观测下肢静脉曲张与肥大细胞浸润及原癌基因c fos表达的关系 ,探索下肢静脉曲张的发病机理。方法 :采用甲苯胺蓝染色及免疫组织化学染色方法对下肢静脉曲张患者的曲张段静脉 2 0例及非曲张段静脉 2 0例标本中的肥大细胞 (MC)数及原癌基因c fos表达分别进行检测。结果 :曲张静脉组的肥大细胞数为(1 4 95± 6 0 8)个 /HP ,明显高于对照组的 (4 9± 2 0 2 )个 /HP ,P <0 0 1。曲张静脉组的c fos阳性血管平滑肌细胞(VSMC)百分率 (75 1± 7 98) %明显高于对照组的 (2 4 2 5± 9 6 1 ) % ,P <0 0 5。下肢静脉曲张静脉壁中的c fos阳性血管平滑肌细胞百分率与肥大细胞数呈直线正相关 ,相关系数r =0 895 7,P <0 0 1。结论 :肥大细胞的浸润及原癌基因c fos的表达在下肢静脉曲张的发生中起作用 ,可能是MC释放各种介质激活VSMC中c fos表达 ,进而促使VSMC表型转变、增殖和迁移 。
Objective: To investigate the role of mast cell infiltration and c fos expression in varices in the lower limbs, and to explore the pathogenesis of the disease. Methods: Immunohistochemical techniques and toluidine blue stain were adopted to study the proto oncogene c fos expression and the influence of mast cell infiltration was studied with twenty segments of varices veins and twenty normal looking segments of the saphenous veins removed from patients with varices veins in the lower limbs. Results: c fos gene expression in vascular smooth muscle cell (VSMC) was significantly higher in varices vein group(75 1±7 98)% than in control group(24 25±9 61)% ( P <0 05). Mast cell number was significantly higher in varices vein group(14 95±6 08)/HP than in control group (4 9±2 02)/HP ( P <0 05). There was a positive correlation between c fos expression and mast cell number in varices veins in the lower limbs( r=0 895 7, P< 0 01 ). Conclusions: Mast cell infiltration and c fos gene expression play an important role in the development of varices in the lower limbs. It is proposed that many kinds of chemical mediators released from mast cells stimulate c fos expression in VSMC, and VSMCs change from quiescent contractile phenotype into proliferative migratory phenotype. It leads to pathologic changes of vein wall and the pathogenesis of varicose veins in the lower limbs.
