摘要
目的:探讨动力髋技术治疗股骨转子间骨折并发症的发生因素。方法:回顾性分析总结1994-01/2002-12采用动力髋技术治疗的124例患者中46例出现术后并发症。结果:46例并发症包括下肢旋转畸形10例,成角畸形1例,髋内、外翻10例,头至转子间距短缩及退钉2例,小转子未复位3例,内固定失败4例,晚期髋内翻3例,髋关节痛3例,膝关节僵硬3例,膝关节疼痛3例,骨折延迟愈合4例。结论:造成并发症的原因是多方面的。主要有手术时机延误导致手术难度的增加,对骨折类型了解分析不充分;手术操作不规范,对所使用器材的设计特点、使用要领不熟悉;术后忽视康复指导和功能锻炼不当以及过早负重。
AIM:To investigate the influencing factors of postoperative complications following the treatment of intertrochanteric fracture after using dynamic hip screw(DHS). METHODS:Retrospective analysis and summary were performed in 124 patients who were treated with DHS from January,1994 to December,2002.The postoperative complications appeared in 46 cases. RESULTS:Among the 46 cases,10 had rotation deformity of lower limbs,1 had angulation deformity of lower limb,10 had varus or valgus hip,2 had curtate distance between femoral head and trochanter or loose screws,3 had no replacement of lesser trochanter,4 had failure of imternal fixation,3 had late varus hip,3 had coxalgia,3 patients had stiff knee joints,3 had pain in knee joints,and 4 had delayed healing. CONCLUSION:There are many reasons for the complications.The main reasons for complications are:more difficult in operation caused by delayed occasion,insufficient comprehension and analysis of the types of fracture,nonstandard operations,unfamiliarity with the characteristics and usage of the instruments and the neglected rehabliltation instruction,inappropriate functional training and early weight loading after operation.
