摘要
目的 探讨肥胖、高脂血症在急性胰腺炎发病中的影响与作用。 方法 选择重症急性胰腺炎 (SAP)患者 98例 ,轻症急性胰腺炎 (MAP)患者 10 7例 ,测定体重指数 (BMI)、血脂水平以及急性胰腺炎相关的实验室检查指标。 结果 肥胖组 (BMI≥ 2 8kg/ m2 )共有 6 4例 ,超重组 (2 4 ≤ BMI<2 8kg/m2 )和体重正常组 (BMI<2 4 kg/ m2 )分别有 6 6例和 75例。肥胖组 SAP比例高于超重组和体重正常组 ;SAP患者 BMI和血三酰甘油高于超重组和体重正常组 ;对于肥胖的急性胰腺炎患者 ,血脂升高伴随着更为严重的胰腺和肝肾功能损害 ,在超重组和体重正常组未观察到同样的效应。 结论 肥胖影响急性胰腺炎的严重程度 ,并且肥胖者发生急性胰腺炎后 ,血脂水平可作为急性胰腺炎严重程度的预测指标。
Objective To investigate the influence of obesity and hyperlipemia on the development of acute pancreatitis(AP). Methods 98 patients with severe acute pancreatitis(SAP) and 107 patients with mild acute pancreatitis (MAP) were included in this study. Obesity was evaluated by measurement of body mass index (BMI). The levels of serum triglyceride and cholesterol were also measured. The impairment of pancreas,liver and kidney functions was assessed according to laboratory evaluations. Results There were 64 patients in group Ⅰ (BMI > 28 kg/m 2 obese);66 in group Ⅱ (BMI > 24 and ≤ 28 kg/m 2,overweight);and 75 in group Ⅲ (BMI ≤ 24 kg/m 2,normal or low weight). The incidence of SAP in group Ⅰ was significantly higher than that of the other two groups;BMI and serum triglyceride of SAP patients were also higher than MAP patients. It was also found that obese AP patients with hyperlipaemia had poorer prognosis of the disease. The same phenomenon was not found in hyperlipaemic patients of group Ⅱ and group Ⅲ. Conclusions The results of this study show that obesity was associated with increased incidence of SAP,and that the level of serum triglyceride in obese patients intensifies the severity of acute pancreatitis. The results suggest that obese patients should be given closer monitoring and more intensive treatments.
