摘要
目的 探讨儿童原发性肾病综合征 (NS)淋巴细胞免疫功能的动态变化 ,检测可溶性白介素 2受体 (sIL - 2R)和T细胞亚群 ,阐明其临床意义。方法 应用酶联免疫吸附试验 (ELISA)方法对 2 5例NS活动期和 2 0例缓解期患儿的血清和尿液检测sIL - 2R浓度。应用流式细胞仪检测T细胞亚群的变化。结果 ①NS活动期组血清和尿液sIL - 2R浓度分别高于缓解期组 (P <0 .0 1 )和对照组 (P<0 .0 1 ) ;②NS活动期组CD3+ 、CD4+ 、CD4+ /CD8+ 比值、NK[CD(1 6 +56) ] + 细胞均明显低于缓解期组 (P <0 .0 1 )和对照组 (P <0 .0 1 )。结论 NS细胞免疫功能减低 ,血和尿中sIL - 2R浓度的变化可作为肾病综合征活动指标之一。
Objective To study the dynamic changes of lymphocytes immune function in children with idiopathic nephrotic syndrome(INS), and illustrate its clinical significance. Methods The levels of sIL-2R in serum and urine were measured by ELISA in 25 INS patients at active stage and 20 at catabasis stage.The T cell subsets were also detected by flow cytometer. Results The levels of sIL-2R in serum and urine at active stage were significantly higher than those at catabasis stage (P<0.01) and also higher than that those in the control(P<0.01).There was no significant difference between the level of sIL-2R in serum and urine at INS catabasis stage and those in the control(P>0.05).The percentage of CD3 + cells and CD4 +/CD8 + cells were significantly lower at INS active stage than those at INS catabasis stage and in the control(both P<0.01). Conclusion They can be used as indexes of nephrotic syndrome activation such as reducing of cellular immunity function , production of cytokine,and elevation of sIL-2R level.
