莪术静脉注射液治疗小儿呼吸道合胞病毒肺炎及其作用原理的研究 被引量:38

Study on Ezhu Intravenous Injection in Treating Infantile Respiratory Syncytial Virus Pneumonia and Its Therapeutical Mechanism

摘要 本文报告莪术静脉注射液治疗呼吸道合胞病毒(RSV)肺炎的临床及实验研究。临床研究以0.04%莪术静脉注射液每日20ml/kg治疗婴幼儿RSV肺炎45例。设对照组20例口服治疗RSV肺炎确有疗效的化瘀汤。结果表明两组疗效相同,全部治愈。两组退热时间、喘憋及哮喘消失时间无统计学差异。实验研究以BaLB/c小鼠为RSV肺炎动物模型。结果表明第5、8天莪术治疗组鼠肺RSV分离率明显低于感染组及葡萄糖对照组(P<0.001)及P<0.0001)。并进行了鼠肺光、电镜病理检查。研究结果表明莪术静脉注射液治疗婴幼儿RSV肺炎有较好疗效且无副作用。其机理可能为(1)抑病毒作用;(2)改善肺部微循环;(3) Ezhu Intravenous Injection (EII) in treating respiratory syncytial virus (RSV) pneumonia was studied. 45 patients in therapeutic group were treated with 0.04% EII in dose of 20ml/kg/day for 7-14days. 20 patients in the control group were treated with Huayu decoction(化瘀汤), its efficacy onRSV pneumonia has been confirmed. The results showed that all the patients of both groups werecured and there were no statistic differences in the average days of clearing up the temperaturedyspnea and wheezing(P>0. 05). Experimental studies showed that the isolation rate of RSV on the5th and 8th day in the therapeutic group were lower than that of infected and glucose control groups.The difference was significant(P<0.001 and P<0.0001). Pathological examination showed that on5th day in infected and control group there were epithelial damages and bronchial lumen obstruction,peri-bronchial and periseptal edema, eosinophilic Ieucocytes and inflammatory infiltrations. Theendothelial cells of alveolar capillaries sweld, platelet aggregations and thrombi occured in its Iumens. On 8th day the platelet aggregations diminished, but cellular in filtrations and damages ofbronchial epithelium still existed. In therapeutic group on 5th day most of the alveolar capillaryendothelial cells were normal with mild edema only. The platelet aggregation were much less thanthe infected and control groups. No peribronchial and peri-septal edema was observed. The resultsof both clinical and experimental studies showed that EII had good therapeutic effect on RSVpneumonia with no adverse reaction.
