脑外伤急性期下丘脑和垂体功能改变及其临床意义 被引量:44

Change of hypothalamic-pituitary function and its clinical significance in head injuries

摘要 本文报告57例脑外伤病人血清类洋地黄物质(EDLS)、催乳素(PRL)、促甲状腺激素(TSH)、生长激素(GH)的测定结果,并对其中29例行垂体兴奋试验,观察 PRL、、TSH、GH 对促甲状腺激素释放激素(TRH)刺激的反应。结果显示:脑外伤后下丘脑和垂体分泌功能有明显变化,变化程度与脑伤严重程度有关。本文着重分析讨论这些变化对判断脑伤程度及估计预后的价值。 Measurements of the serum level of endogenousdigitalis-like substance (EDLS). prolactin (PRL).growth hormone (GH). and thyroid stimulatinghormone (TSH) were made in 57 patients withhead injure within 24 hours. The responses of PRL.GH. and TSH to thyrotropin-releasing homone (TRH)were observed in 29 of these cases. The meanlevels of serum EDLS and PRL in all groups wereelevated statistically, compared with control group.The mean of serum GH and TSH were highersignificanlty in Glosgow coma scole (GCS) 8-groupdead group. and dead group with sevre head injury.The responses on serum PRL and TSH to TRH wereabnormal in most of GNS. 8-group and head group.These findings suggest that measurments of EDLSand pituitary hormons and the TRH provocation testare useful to evaluate the severity of head injuryand to predict outcome of patients with headinjury.
