异丙酚复合硬膜外麻醉对静脉用麻黄素升压反应的影响

摘要 目的 观察异丙酚复合硬膜外麻醉对静脉用麻黄素升压反应的影响。方法  2 0例ASAⅠ~Ⅱ级择期行上腹部手术的病人 ,分别于入室清醒时、异丙酚诱导插管后、异丙酚复合硬膜外麻醉后均静脉用麻黄素0 .1mg kg ,观察病人心率 (HR)、收缩压 (SBP)、舒张压 (DBP)、平均动脉压 (MAP)的变化。结果 麻黄素使清醒病人的心率显著增快 (P <0 .0 5 ) ;MAP短暂升高 ,而异丙酚麻醉后 ,麻黄素对病人心率影响不大 ,但升压反应明显增强 (P <0 .0 5 )。与单纯异丙酚麻醉相比 ,异丙酚复合硬膜外麻醉时 ,麻黄素的升压反应并无明显差别 (P >0 .0 5 )。结论 异丙酚增强麻黄素的升压反应 。 Objective To investigate the effects of propofol combined with epidural anesthesia on pressor response to intravenous ephedrine.Methods Twenty adult patients of ASA gradeⅠ~Ⅱ undergoing selective operation received propofol combined with epidural anesthesia.All patients received ephedrine 0.1mg/kg IV before and after the anesthetic induction,after propofol combined with epidural anesthesia.The changes of HR,SBP,DBP and MBP were monitored.Results Ephedrine increased the heart rate significantly (P<0.05) in wake patients.In contrast,there was no increase in heart rate after the ephedrine administration under propofol anesthesia and propofol combined with epidural anesthesia.We also observed that the magnitudes of the pressor responses to ephedrine during propofol anesthesia and propofol combined with epidural anesthesia were significantly greater(P<0.05) than during the awake state.However,the pressor response to intravenous ephedrine was not significantly different between propofol anesthesia and propofol combined with epidural anesthesia.Conclusion Propofol anesthesia augment the pressor responses to intravenous ephedrine.Ephedrine is effective to reverse the hypotention induced by propofol combined with epidural anesthesia.
