Sepsis is a life-threatening condition caused by a dysregulated response of the body in response to an infection that harms its tissues and organs.Interleukin-6(IL-6)is a significant component of the inflammatory response as part of the pa-thogenesis of sepsis.It aids in the development of Acute lung injury and,subse-quently,multiple organ dysfunction syndrome.This letter probes into the corre-lation between plasma IL-6 levels and the risk of developing acute lung injury and multiple organ dysfunction syndrome in critically ill patients with sepsis.While it shows promising results,limitations like its observational study design,a limited sample size,a single center involvement,single-time-point measurement,and a lack of a control group restrain its cogency.The study is a big step in identifying IL-6 as a biomarker to improve patient care. 展开更多

Autism spectrum disorder(ASD)is a complex neurodevelopmental disorder affecting over 2%of the global population,marked by social communication deficits and repetitive behaviors.Kabatas et al explored the efficacy and safety of Wharton’s jelly-derived mesenchymal stem cell(WJ-MSC)therapy in a 4-year-old child with ASD.Using the childhood autism rating scale and Denver II develop-mental screening test,significant improvements were seen after six WJ-MSC sessions,with no adverse events over 2 years.Despite promising results,the study’s single-case design limits generalizability.Larger,multi-center trials are needed to validate the findings and assess long-term effects of WJ-MSC therapy in ASD. 展开更多

Coronavirus disease 2019(COVID-19)is a contagious disease caused by severe acute respiratory syndrome coronavirus 2.It was declared a global pandemic on March 11,2020,by the World Health Organization.An excessive inflammatory response is a severe respiratory manifestation of COVID-19,which becomes predominant in later stages.Due to its immunosuppressive and anti-inflammatory properties,dexamethasone is the first systemic glucocorticoid to treat severe COVID-19 patients.This editorial reviews the efficacy and safety of highdose vs low-dose dexamethasone in patients with COVID-19.Findings indicate that using low-dose dexamethasone is beneficial and emphasize the need for additional research on the use of high-dose dexamethasone.While the study provides a robust evidence base,it is limited by the lack of long-term data,focus on specific outcomes and heterogeneity of the included studies.Future research should focus on the long-term effects of dexamethasone and its impact across varying disease severities and patient populations to refine treatment strategies and improve patient care. 展开更多

Blepharoplasty is a frequently performed aesthetic surgery today aimed at enhancing eyelid appearance and correcting age-related changes.The traditional method of subtraction blepharoplasty,which involved removing fat and excess skin,is now considered outdated.This letter explores Gorgy et al's commentary on Miotti et al's study,highlighting a shift in upper eyelid blepharoplasty towards a more conservative,volume-preserving approach.The study systematically reviewed 10 publications,including three retrospective studies,five comparative studies,and two clinical trials.It emphasizes the trend towards preserving the patient's natural anatomy and focusing on enhancement rather than alteration.However,the study's limitations,such as the lack of long-term comparative re-search,a relatively small sample size,and a single-center design,indicate that further research with extended follow-up is necessary to validate the safety and effectiveness of these techniques.The focus is increasingly on preserving and augmenting volume in upper blepharoplasty rather than removing tissue. 展开更多

BACKGROUND Gallbladder stones are a common occurrence,with a prevalence of approximately 10%in the Pakistani population.A rare but potentially fatal complication of gallstones is cholecystogastric fistulas.The underlying mechanism involves chronic inflammation due to cholelithiasis,causing gradual erosion and eventually leading to fistula formation.CASE SUMMARY We present a rare case of a cholecystogastric fistula in a 40-year-old female patient,successfully managed with an open surgical approach.The patient initially presented with a 6-month history of intermittent epigastric pain,nausea,and vomiting,which worsened over time.Laboratory investigations and abdominal ultrasound confirmed cholelithiasis,and laparoscopic cholecystectomy was planned.However,intraoperative findings revealed a cholecystogastric fistula,a rare complication of chronic gallstone disease.Given the dense adhesions between the gallbladder and the stomach,the procedure was converted to an open surgery.The fistula was divided,and a cholecystectomy was performed,along with primary repair of the gastric defect using a double-layer suture and reinforcement with an omental patch.The patient recovered uneventfully and was discharged on the third postoperative day.CONCLUSION This case highlights the importance of considering cholecystogastric fistula in patients with vague gastrointestinal symptoms and chronic cholelithiasis.The report discusses diagnostic challenges,surgical approaches,and a review of the current literature on managing such rare but serious complications of gallstones. 展开更多

Hemorrhoids are a common and painful condition,with conventional treatments such as endoscopic rubber band ligation(ERBL)and injection sclerotherapy often falling short due to high recurrence rates and significant post-operative pain.A clinical trial by Qu et al introduces a novel approach called endoscopic polidocanol foam sclerobanding(EFSB).This multicenter randomized trial involved 195 patients with grade II and III internal hemorrhoids and demonstrated that EFSB significantly reduced recurrence rates and post-procedural pain while improving symptom relief and patient satisfaction compared to ERBL.The study's strengths include its robust design,comprehensive outcome evaluation,and patient-centered approach.Despite limitations such as the single-blind design and relatively short follow-up period,the findings suggest that EFSB could enhance clinical practice by offering a more effective and patient-friendly treatment option.Further research is needed to validate these results and explore the long-term benefits and cost-effectiveness of EFSB. 展开更多

Alzheimer’s disease(AD)is a grave illness that results in cognitive and social issues.A recent study examined the association between neuroimaging results,cognitive dysfunction,atypical cellular immune function,and poor prognostic factors in AD patients who demonstrated poor prognosis.Poor prognosis was associated with abnormal cellular immune function,extrapyramidal symptoms,altered consciousness,abnormal electroencephalogram,modified Rankin scale,increased neutrophil lymphocyte ratio,and severe pneumonia.The impaired cellular immune function characterized by a reduction in the blood T lym-phocytes’proportion predicted poor prognosis as an independent risk factor in AD.Early initiation and maintenance of AD medications is associated with better outcomes. 展开更多

The gut microbiome is an extensive variety of bacteria with a range of metabolic capabilities that can be pathogenic,beneficial,or opportunistic.Changes in the gut microbiota's composition can affect the link between gut integrity and host health as well as cause disruptions to numerous neurological systems.The second most prevalent mental health problem,insomnia has a negative social and economic impact.Currently,it is becoming increasingly obvious how crucial it is to preserve the delicate balance of gut microbiota to treat illness-related symptoms like insomnia.Although traditional Chinese medicine has proposed an effective strategy against insomnia through gut microbiota alteration in animal models,studies in human models are limited.This decreases the predictive value of the studies in terms of human outcomes.This editorial places an emphasis on cultural sensitivity rather than scientific reasoning that promotes the use of traditional Chinese medicine(TCM).We aim to emphasize the concern that promoting TCM could divert resources from conventional medical research,leading to suboptimal care. 展开更多

Diabetes is one of the most devastating medical dilemmas impacting every region of the world severely.The study by Tian et al investigates glymphatic system dys-function in the context of glucose metabolism and diabetes,using diffusion tensor imaging along the perivascular space.The study evaluated individuals with type 2 diabetes mellitus(T2DM),prediabetes,and normal glucose metabolism.It found that prediabetic and T2DM groups had significantly impaired glymphatic func-tion.Glymphatic dysfunction may serve as an early indicator of cognitive deteri-oration in diabetes due to the correlations shown between these abnormalities and clinical factors as well as cognitive performance.The study has some positives,such as thorough evaluations and novel imaging methods,but its cross-sectional design and limited sample size restrict its applicability.More extensive,long-term research is required to verify these results.Furthermore,there are significant cli-nical implications.Patients with diabetes may benefit from immediate therapies to prevent microvascular and macrovascular damage if glymphatic dysfunction is identified early.The study promotes comprehensive diabetes care with a focus on maintaining cognitive function.In conclusion,the work of Tian et al is crucial because it opens the door to better treatment and diagnostic strategies for dia-betes-related cognitive deterioration. 展开更多

Surgical approach for lower postoperative anal stenosis

After anorectal surgery,hemorrhoids frequently lead to chronic issues,partic-ularly in patients with mixed hemorrhoids.Liu et al investigated the outcomes of staple removal at the 3-and 9-o’clock positions following modified stapled hemorrhoidopexy(SH)in patients with grade III or IV hemorrhoids.This study included patients who underwent standard or modified SH between January 1,2015,and January 1,2020.Key metrics assessed included hospital stay duration,blood loss,operation time,and the incidence of minor or major complications.The findings indicated that the modified SH technique is a safe option for advanced-grade hemorrhoids,resulting in a lower rate of postoperative anal stenosis compared to standard SH.Notably,this technique also showed reduced anal stenosis rates in patients with prior hemorrhoid treatments.While the modified SH demonstrates immediate benefits,further research is necessary to evaluate long-term effects.Despite its advantages,the study’s limited sample size restricts the generalizability of the findings,underscoring the need for larger,long-term studies to validate these results.Clinically,the modified SH method appears to significantly reduce the incidence of postoperative anal stenosis,a common concern following typical surgeries.If confirmed by larger trials,this procedure may become the preferred surgical approach for hemorrhoids.In conclusion,the work of Liu et al signifies a meaningful advancement in hemorrhoid surgery,enhancing patient safety and outcomes. 展开更多
