Analysis on the Effect of Yiqi Huoxue Decoction Combined with Neuromuscular Electrical Stimulation in Improving ICU-Acquired Debility in Mechanically Ventilated Patients

Objective:To investigate the effect of Yiqi Huoxue decoction combined with neuromuscular electrical stimulation on improving intensive care unit(ICU)acquired debility in mechanically ventilated patients.Methods:50 patients who were admitted to the ICU and received mechanical ventilation treatment in our hospital from June 2022 to June 2023 and were complicated with ICU-acquired neurasthenia were selected,and randomly grouped using the randomized envelope method into two groups:control group with 25 patients who received neuromuscular electrical stimulation alone;observation group with 25 patients who received the traditional Chinese medicine Yiqi Huoxue decoction.Comparison indexes:treatment efficiency,degree of emotional recovery(APACHEⅡscore),muscle strength status(MRC score),motor status(FAC rating),and self-care ability(BI index score).Results:The treatment efficiency of patients in the observation group patients was higher as compared to those in the control group(P<0.05).There was no significant difference in the comparison of the results of the scores(ratings)of each index between the two groups before treatment(P>0.05).After the treatment,the APACHEⅡscores of patients in the observation group were significantly lower as compared to those in the control group,while the MRC scores,FAC ratings,and BI index scores were higher in the observation group than those of the control group patients(P<0.05).Conclusion:The combined application of Yiqi Huoxue decoction and neuromuscular electrical stimulation in the treatment of patients with ICU-acquired neurasthenia complicated by mechanical ventilation significantly enhanced the clinical efficacy,the patient’s muscle strength,motor status,and ability of self-care.Hence,it has high application value and is worthy to be popularized. 展开更多

Effect of Yiqi Huoxue (益气活血) Herbs on Vascular Endothelial Cells and Platelets in Patients with Chronic Cor Pulmonale

Objective: To observe the clinical effect of Yiqi Huoxue (益气活血, YQHX) herbs in treating the patients with chronic cor pulmonale and to explore its mechanism by determining the relationship of oxida-tion/antioxidation system and how such herbs change on the function of endothelial cells and platelets. Methods: Fifty-eight patients were divided into two groups: conventional therapy group (control group, 28 patients) and convention plus YQHX herbs group (treated group, 30 patients). The control group received conventional management. The treated group were treated with YQHX 150 ml, twice a day, plus the conventional treatment, and the clinical efficacy was recorded. The lipid peroxidation (LPO), superoxide dismutase (SOD), ot-granule membrane protein (GMP140), partial pressure of arterial oxygen (PaO2), partial pressure of carbon dioxide in artery (PaCO2) and circulating endothelial cells (CEC) were measured respectively before and after treatment, and the relationship between various parameters were analyzed. The results were compared with those of 10 healthy subjects got at the same period. Results: (1) The effective rate and PaO2 of the treated group was higher than that of the control group and there were no difference in PaCO2 between the two groups. (2) The levels of LPO, GMP140, CEC in all the patients before therapy were significantly higher than those of the healthy group, and there were marked decrease in the levels of those after treatment (all P<0. 01). On the contrary, the levels of SOD in all the patients before therapy were markedly lower than those in the healthy subjects and increased after treatment, P<0. 01. (3) The increase of SOD in the treated group was significantly more obvious than that of the control group. In the treated group, the decrease of LPO, GMP140, CEC were markedly more obvious than those in the control group (all P<0. 01). (4) The number of CEC, as well as GMP140, was negatively correlated to PaO2(P<0.01) and SOD (P< 0. 01), which was positively correlated to LPO (P<0. 01). There was a positive correlation between CEC and GMP140 (P<0.05). Conclusion: YQHX herbs in treating chronic cor pulmonale proved to be effective by balancing the oxidation and antioxidation, protecting the pulmonary endothelial cells and activated platelets and helpful in treating respiratory failure. 展开更多

目的探讨益气活血化浊解毒方调控KAT3B/STING轴介导的小胶质细胞极化对缺血性脑卒中大鼠神经功能的影响。方法复制缺血性脑卒中大鼠模型。将复制成功的60只大鼠分为缺血性脑卒中组、益气活血化浊解毒方低剂量组(低剂量组)、益气活血化... 目的探讨益气活血化浊解毒方调控KAT3B/STING轴介导的小胶质细胞极化对缺血性脑卒中大鼠神经功能的影响。方法复制缺血性脑卒中大鼠模型。将复制成功的60只大鼠分为缺血性脑卒中组、益气活血化浊解毒方低剂量组(低剂量组)、益气活血化浊解毒方高剂量组(高剂量组)、益气活血化浊解毒方高剂量+2,5-己酮可可碱(DMXAA)组(高剂量+DMXAA组),每组15只。另取15只正常大鼠为Sham组。低剂量组大鼠灌胃0.2 mL 27.5 g/mL的益气活血化浊解毒方药液,高剂量组大鼠灌胃0.2 mL 55.0 g/mL的益气活血化浊解毒方药液,高剂量+DMXAA组大鼠灌胃0.2 mL 55.0 g/mL的益气活血化浊解毒方药液和25 mg/kg的DMXAA,Sham组和缺血性脑卒中组给予等体积生理盐水代替药物;除高剂量+DMXAA组外,其余组大鼠再给予等体积二甲基亚砜溶液灌胃,1 d/次,连续4周。Zea-Longa评分和网屏试验评分评估大鼠神经功能和神经行为学;检测大鼠脑组织含水量;TTC染色法检测大鼠脑梗死面积;酶联免疫吸附试验检测大鼠血清LDH和NGF水平和缺血侧皮层组织TNF-α、IL-6、IL-10水平;HE染色观察大鼠缺血侧皮层组织病理变化;Western blotting检测大鼠缺血侧皮层组织iNOS、Iba1、CD68、CD40、CD206、Arg-1、Cleaved caspase-3、KAT3B、STING蛋白表达。结果缺血性脑卒中组大鼠Zea-Longa评分和网屏试验评分、脑组织含水量、脑梗死面积百分比均高于Sham组(P<0.05),低剂量组和高剂量组大鼠Zea-Longa评分和网屏试验评分、脑组织含水量、脑梗死面积百分比均低于缺血性脑卒中组(P<0.05),高剂量+DMXAA组大鼠Zea-Longa评分和网屏试验评分、脑组织含水量、脑梗死面积百分比均高于高剂量组(P<0.05)。缺血性脑卒中组大鼠NGF和IL-10水平低于Sham组,LDH、TNF-α和IL-6水平高于Sham组(P<0.05);低剂量组和高剂量组大鼠NGF和IL-10水平高于缺血性脑卒中组,LDH、TNF-α和IL-6水平低于缺血性脑卒中组(P<0.05);高剂量+DMXAA组大鼠NGF和IL-10水平低于高剂量组,LDH、TNF-α和IL-6高于高剂量组(P<0.05)。与缺血性脑卒中组比较,低剂量组和高剂量组缺血侧皮层神经元形态有明显改善。缺血性脑卒中组大鼠缺血侧皮层组织中iNOS、Iba1、CD68和CD40蛋白相对表达量比Sham组高,CD206和Arg-1蛋白相对表达量比Sham组低(P<0.05);低剂量组和高剂量组大鼠缺血侧皮层组织中iNOS、Iba1、CD68和CD40蛋白相对表达量比缺血性脑卒中组低,CD206和Arg-1蛋白相对表达量比缺血性脑卒中组高(P<0.05);高剂量+DMXAA组大鼠缺血侧皮层组织中iNOS、Iba1、CD68和CD40蛋白相对表达量比高剂量组高,CD206和Arg-1蛋白相对表达量比高剂量组低(P<0.05)。结论益气活血化浊解毒方可调节缺血性脑卒中大鼠小胶质细胞极化,减轻神经炎症和神经损伤,改善神经功能,可能与抑制KAT3B/STING轴有关。 展开更多

基于营卫理论探讨益气活血通脉汤治疗气虚血瘀型反杓型高血压的临床研究

目的:探讨基于营卫理论指导下应用益气活血通脉汤治疗气虚血瘀型反杓型高血压的疗效。方法:选取2022年1—2月就诊于江西中医药大学附属医院心血管科的气虚血瘀型反杓型高血压患者60例,随机分为对照组和观察组,各30例。对照组给予苯磺酸... 目的:探讨基于营卫理论指导下应用益气活血通脉汤治疗气虚血瘀型反杓型高血压的疗效。方法:选取2022年1—2月就诊于江西中医药大学附属医院心血管科的气虚血瘀型反杓型高血压患者60例,随机分为对照组和观察组,各30例。对照组给予苯磺酸氨氯地平片治疗,观察组在对照组基础上给予益气活血通脉汤治疗,疗程为12周。观察2组患者临床疗效,治疗前后中医证候积分、血脂水平[总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)]、动态血压水平[24 h平均收缩压(24 h SBP)、24 h平均舒张压(24 h DBP)、日间平均收缩压(dSBP)、日间平均舒张压(dDBP)、夜间平均收缩压(n SBP)、夜间平均舒张压(n DBP)、夜间收缩压下降率(SBPF)、夜间舒张压下降率(DBPF)、收缩压负荷(SBPL)、舒张压负荷(DBPL)、晨峰血压(MBPS)、脉压差(PP)]、血压昼夜节律,并记录不良反应。结果:治疗后,观察组总有效率(96.67%)高于对照组(73.33%)(P<0.05);2组患者治疗后中医证候积分、血脂水平均较治疗前下降(P<0.05,P<0.01),且观察组中医证候积分及TC、TG水平均低于对照组(P<0.05);2组患者治疗后各动态血压水平均较治疗前降低,且观察组24 h SBP、n SBP、24 h DBP、dDBP、24 h SBPL、d SBPL、nSBPL、24 h DBPL、dDBPL、MBPS均低于对照组(P<0.05);治疗后观察组反杓型血压昼夜节律逆转率高于对照组(P<0.05);治疗期间2组患者的不良反应无显著差异。结论:营卫理论指导下的益气活血通脉汤能够缓解气虚血瘀型反杓型高血压患者临床症状,降低患者血压,恢复血压节律。 展开更多

益气活血解毒方灌肠在瘀毒互结型高脂血症性胰腺炎的应用观察

目的:探讨益气活血解毒方灌肠在瘀毒互结型高脂血症性胰腺炎的临床效果。方法:选择2022年7月—2023年10月于如皋市中医院就诊的瘀毒互结型高脂血症性胰腺炎(HTG-AP)患者84例为研究对象,采用随机数字表法分成常规组与联合组,每组42例。... 目的:探讨益气活血解毒方灌肠在瘀毒互结型高脂血症性胰腺炎的临床效果。方法:选择2022年7月—2023年10月于如皋市中医院就诊的瘀毒互结型高脂血症性胰腺炎(HTG-AP)患者84例为研究对象,采用随机数字表法分成常规组与联合组,每组42例。常规组采用常规西医治疗,联合组采用益气活血解毒方灌肠治疗。治疗后,观察两组患者的实验室指标、中医证候积分、胃肠功能恢复情况、临床疗效与不良反应。结果:治疗后,联合组患者中医证候积分低于常规组(P<0.01);联合组血清淀粉酶(AMY)、胰脂肪酶(LPS)、超敏C反应蛋白(hs-CRP)、降钙素原(PCT)水平均低于常规组(P<0.01);联合组患者腹胀缓解时间、肠鸣音恢复时间、首次排气时间、首次排便时间均低于常规组(P<0.05,P<0.01);联合组患者治疗总有效率高于常规组(P<0.05);两组不良反应总发生率差异无统计学意义(P>0.05)。结论:益气活血解毒方灌肠能够缓解临床症状,改善患者实验室指标,降低中医证候积分,缩短患者胃肠功能恢复时间,提高临床疗效,安全性较高。 展开更多

【目的】探究益气活血通脉汤(补阳还五汤为主化裁)联合西药对左心疾病相关性肺动脉高压(PH-LHD)气虚血瘀证患者的疗效及内皮功能的影响。【方法】将2022年3月~2023年12月在商洛市中心医院接受治疗的80例PH-LHD气虚血瘀证患者随机分为研... 【目的】探究益气活血通脉汤(补阳还五汤为主化裁)联合西药对左心疾病相关性肺动脉高压(PH-LHD)气虚血瘀证患者的疗效及内皮功能的影响。【方法】将2022年3月~2023年12月在商洛市中心医院接受治疗的80例PH-LHD气虚血瘀证患者随机分为研究组和对照组,每组各40例。对照组给予西医常规治疗,研究组在对照组的基础上加用益气活血通脉汤治疗,疗程为3周。观察2组患者干预前后中医证候积分、肺循环指标[平均肺动脉压(mPAP)和肺毛细血管楔压(PCWP)]、心肺运动试验(CPET)指标[峰值心率、峰值收缩压、峰值舒张压、峰值氧耗量(VO2max)、峰值氧脉搏]、血管内皮指标[血清一氧化氮(NO)和内皮素1(ET-1)]及生活质量的变化情况,比较2组患者的临床疗效和不良反应情况。【结果】(1)疗效方面,干预3周后,研究组的总有效率为90.00%(36/40),对照组为72.50%(29/40),组间比较(χ²检验),研究组的疗效明显优于对照组(P<0.05)。(2)中医证候积分方面,干预后,2组患者的主症积分、次症积分及总积分均较干预前降低(P<0.05),且研究组的降低幅度均明显优于对照组(P<0.01)。(3)肺循环指标方面,干预后,2组患者的mPAP、PCWP水平均较干预前降低(P<0.05),且研究组的降低幅度均明显优于对照组(P<0.01)。(4)CPET指标方面,干预后,2组患者的峰值心率、峰值收缩压和峰值舒张压均较干预前降低(P<0.05),VO2max和峰值氧脉搏均较干预前升高(P<0.05),且研究组对峰值心率、峰值收缩压和峰值舒张压的降低幅度及对VO2max和峰值氧脉搏的升高幅度均明显优于对照组(P<0.05或P<0.01)。(5)血管内皮指标方面,干预后,2组患者的血清ET-1水平均较干预前降低(P<0.05),血清NO水平均较干预前升高(P<0.05),且研究组对血清ET-1水平的降低幅度及对血清NO水平的升高幅度均明显优于对照组(P<0.01)。(6)生活质量方面,干预后,2组患者健康调查简表(SF-36)的精神健康、情感职能、社会功能、生理功能、活力、总体健康等6个领域的生活质量评分均较干预前升高(P<0.05),且研究组的升高幅度均明显优于对照组(P<0.05或P<0.01)。(7)不良反应方面,研究组的不良反应发生率为20.00%(8/40),对照组为15.00%(6/40),组间比较,差异无统计学意义(P>0.05)。【结论】益气活血通脉汤与西药联合应用能有效提高PH-LHD气虚血瘀证患者临床疗效,改善患者心肺功能及血管内皮功能,提高患者生活质量,且具有较高的安全性。 展开更多

Effect of Bushen Yiqi Huoxue Recipe on Placental Vasculature in Pregnant Rats with Fetal Growth Restriction Induced by Passive Smoking 被引量:5

Interactions of vascular endothelial growth factor (VEGF) with receptors VEGFR1/Fltl and VEGFR2/Flk1, and those of angiopoietins (Ang-1, Ang-2) with receptor Tie2 play important roles in placental angiogenesis. This study investigated vascular morphology and expression of these angiogenic factors in rat placenta on the day 15, 18, 21 of gestation (D 15, D 18 and D21). The rats were randomly assigned into 3 groups: normal group, model group [fetal growth restriction (FGR) model], and Bushen Tqi Huoxue (BYHR) recipe treatment group (BYHR group, the pregnant rats with FGR were treated with BYHR recipe). Morphological analysis indicated that during initial villous formation, fetal nucle- ated erythrocytes (FNEs) appeared in maternal blood sinus (MBS). Subsequently, FNEs were sur- rounded by endothelial cells to form fetal capillary (FC) and then by trophoblast cells to form villi. As pregnancy proceeded, FC density increased progressively with increasing endothelial identification staining (EIS) in normal and BYHR groups. Whereas, villous formation was suppressed, normal in- crease in FC density was impaired and EIS was weakened in model group. Quantitative PCR analysis showed that VEGF and Flkl mRNA increased over gestation in all groups, indicating that VEGF might play a pivotal role in FC growth during late gestation. VEGF mRNA was increased on D15, while de- creased on D21 in model group as compared with normal group and BYHR group. Immunohistochemi- cally, Ang-2 protein was highly expressed in FNEs, gradually disappeared as villi matured, and decreased over gestation in all groups, indicating that Ang-2 might play a pivotal role in villous formation, which was further supported by decreased Ang-2 mRNA and protein expression in model group on D 15. Ang-1 mRNA, Tie2 mRNA and Ang-1/Ang-2 ratio increased from D15 to D18 in all groups as placenta matured. Ang-1 mRNA, Tie2 mRNA and Ang-1/Ang-2 ratio were decreased on D18 in model group as compared with normal and BYHR groups, indicating delayed maturity of FGR placenta. Alterations in angiogenic factors may result in altered placental vasculature and cause placental insufficiency. BYHR recipe could balance the angiogenic factors to promote the formation and maturation of FGR placental vasculature. 展开更多

BACKGROUND Early diabetic nephropathy(DN)is a complication of diabetes mellitus.It mainly affects kidney microvessels and glomerular function,and its timely and effective treatment is critical for early DN.However,the effects of treatments comprising simple Western medicine are not optimal.With the promotion and implementation of integrated Chinese and western medicine treatments,remarkable results have been achieved for many diseases.To this end,we explored the clinical efficacy of integrated traditional Chinese and western medicines for the treatment of early DN.AIM To investigate the effect of sitagliptin tablets combined with Yiqi yangyin huoxue decoction on clinical efficacy and hemorheology in patients with early DN.METHODS Through a retrospective analysis,123 patients with early DN were admitted to the endocrinology clinic of the Changzhou NO.7 People’s Hospital from January 2021 to October 2022 and were selected as study subjects.After rigorous screening,100 patients with early DN were enrolled.The control group(CG,n=50)and the observation group(OG,n=50)were divided according to the treatment method.The CG were treated with sitagliptin,and the OG were treated with sitagliptin plus the Yiqi yangyin huoxue decoction.Both groups were treated for 3 mo.For both groups,the baseline data and clinical efficacy were compared,and changes in blood glucose levels,lipid levels,renal function,and hematological indicators before(T0)and after(T1)treatment were assessed.RESULTS The total effective rate for the OG was 94.00%and that of the CG was 80.00%(P<0.05).After treatment(T1),the levels of fasting blood glucose,2 h postprandial glucose,total cholesterol,triacylglycerol,and low-density lipoprotein cholesterol in OG patients were obviously lower than those in the CG(P<0.05),and cystatin C,homocysteine,urinary microalbumin,and blood creatinine values in OG patients were also obviously lower than those in the CG(P<0.05);erythrocyte deposition,plasma viscosity,whole blood high shear viscosity,and whole blood low shear viscosity were markedly lower in OG patients than in the CG(P<0.05).CONCLUSION Sitagliptin combined with Yiqi yangyin huoxue decoction has a remarkable effect when used to treat patients with early DN.Further,it is helpful in improving hemorheological indices and controlling disease progression. 展开更多

Objective:Yiqi Huoxue Decoction (YQHX) has been widely used for clinical treatment of ischemic heart disease.While oxidative stress plays a key role in the pathogenesis of ischemic heart disease,the function and molecular mechanism underlying antioxidative protective effects of YQHX on H9c2 cardiomyocytes against ischemia/hypoxia (I/H) have yet to be well clarified.Methods:H9c2 cells were subjected to 12 h of hypoxia with serum-free conditions and then treated with or without YQHX (100-400 μg/mL).Cell viability was examined using a CCK-8 assay.Maleic dialdehyde (MDA) and superoxide dismutase (SOD) activity were detected using commercial kits.Intracellular reactive oxygen species (ROS) levels and mitochondrial membrane potential were measured using fluorescence microscopy and confocal laser-scanning microscopy,respectively.Ultrastructural details of mitochondria in H9c2 cells were observed using transmission electron microscopy.The antioxidative protective pathway was assessed by measuring mRNA and protein expression of Nrf2 and HO-1,as well as AMPK activation.Results:I/H injury gradually induced oxidative stress.Treatment with YQHX significantly increased cell viability and reversed I/H-induced oxidative stress,including reducing the production of oxidative stress products (ROS and MDA),increasing SOD levels,improving mitochondrial morphology,and increasing mitochondrial membrane potential.YQHX was also observed to increase I/H-induced expression of Nrf2 and HO-1,and the activation effects of YQHX were blocked by an AMPK inhibitor.In addition,HPLC analysis showed that YQHX contained two active antioxidative constituents (calycosin and ferulic acid).Conclusion:The results suggest that anti-oxidative effects exerted by YQHX in H9c2 cardiomyocytes may be linked to upregulation of the AMPK-mediated Nrf2/HO-1 pathway. 展开更多

Objective:To investigate the cardioprotective effect of Yiqi Huoxue granule(YQHXG)in the regulation of autophagy in rats induced with myocardial infarction(MI).Methods:An acute MI animal model was established by ligation of the left anterior descending branch of the coronary artery in Sprague-Dawley rats.Besides,20 rats received sham operation were classified into a control group.The remaining 59 rats were randomly divided into MI model group(n=19),YQHXG group(n=20),and perindopril group(n=20).Relevant indicators on days 7 and 28 were observed in each group.Left ventricular function was determined by echocardiography.The structure and morphology of mitochondria,and the number of autophagic vesicles,were observed by transmission electron microscopy.The mRNA and protein expression levels of LC3,FUNDC1,Beclin-1,and BNIP3 were examined in the tissue of the MI marginal area.Results:Compared with the MI model group,YQHXG showed obvious improvements in cardiac functions.Observing the microscopic morphology of the heart tissue,myocardial tissue damage attenuated,autophagic signs of autophagosomes and autolysosomes reduced,vacuolization in mitochondria mitigated,and mitochondria arranged in order.YQHXG could reduce the degree of tissue lesion after MI and regulate the expression of autophagy-related molecules at different stages.On Day 7,YQHXG significantly downregulated the expression of Fundc1,Becn1,Bnip3 mRNA and reduced the levels of FUNDC1,Beclin-1,BNIP3,and LC3 B proteins expression(all P<.001).On Day 28,YQHXG could upregulate the expression of Becn1,Fundc1 and Bnip3 mRNA and increased the levels of the corresponding proteins expression(all P<.001).Besides,it also increased LC3 B protein expression level(P=.0344).Conclusion:YQHXG regulated the expression of mitochondrial autophagy-related factors in myocardial tissue and mitochondrial autophagic activity at different stages to protect the heart following MI. 展开更多

Background:In myocardial ischemia,hypoxia leads to destruction of the cytoskeleton,and especially the imbalance of microtubule polymerization-depolymerization,which seriously affects the structure and function of cardiomyocytes.We previously showed that a Yiqi Huoxue Decoction (YQHX) improves mitochondrial function and decreases anti-oxidative effects in hypoxia-induced H9c2 cell injury.Therefore,in this study we investigated whether YQHX protects against hypoxia-induced damage by decreasing damage to the cardiac cytoskeleton.Methods:After reaching 70%-80% confluence,H9c2 cells were synchronized in serum-free Dulbecco's Modified Eagle Medium for 6 hours,then divided into control,model,and YQHX (100,200,400 μg/mL) groups,which were then grown in a hypoxic atmosphere for 12 hours.Cardiac cell viability was assessed using an xCELLigence system.The levels of lactate dehydrogenase,maleic dialdehyde,and superoxide dismutase in H9c2 cell supernatants were measured.Hoechst 33258 staining was employed to observe cardiac cell apoptosis.Confocal microscopy,immunofluorescence,and western blot analysis were performed to evaluate the protective effects of the YQHX against hypoxia-induced injury in the H9c2 cell line.Results:Cells that were pretreated with YQHX were more able to maintain their microtubule structure in the early stages of hypoxia and had better myocardial fitness in response to hypoxia compared with cells that were not pretreated.However,hypoxia-induced upregulation of α-tubulin and β-tubulin expression antagonized the protective effect of YQHX (100 μg/mL).In addition,YQHX (100 μg/mL) treatment significantly upregulated MAP4 protein expression (P =.003) and downregulated p-AMPKα protein expression (P <.001) compared with the model group.Conclusion:The results indicate that YQHX plays a role in protecting against oxidative stress injury and apoptosis in H9c2 cells.Notably,our results suggested that the YQHX could mitigate the damage to the cardiac cytoskeleton and the dysregulation of AMPK-related protein signaling pathways that are induced by hypoxia. 展开更多

Effects of yiqi yangyin huoxue therapy on IgA nephropathy: a systematic review and meta-analysis of randomized controlled trials 被引量:1

Objective To assess the efficacy and safety of Yiqi Yangyin Huoxue therapy (YYHT) for the treatment of IgA nephropathy. Methods Databases articles about YYHT in treating IgAN patients were searched through February 2018. Comparisons were YYHT alone or YYHT in combination with western medicine as experimental intervention, and western medicine as the control. Randomized controlled trials (RCTs) were selected. Cochrane collaboration tool was used for assessing risk of bias to evaluate methodologies. RevMan 5.3 software was applied to analyze the data of included trials. Results A total of 21 studies involving 1473 patients were included. Meta-analysis results demonstrated that YYHT alone or YYHT in combination with western medicine were more efficacious than only western medicine in decreasing proteinuria [MD =-0.41, 95% CI (-0.53,-0.29), P < 0.00001] and the urine red blood cells [MD =-7.46, 95% CI (-9.28,-5.64), P < 0.00001]. There also showed improvement in the clinical total effective rate [OR = 2.78, 95% CI (2.14, 3.61), P < 0.00001] between the two groups. Some of the included trials did not report the adverse events. Conclusions Current evidence shows that YYHT can reduce proteinuria and urine red blood cells and improve the total efficacy rate. Since poor quality and publication bias of some included trials, more evidence-based medicine studies needed to define the role of YYHT in the treatment of IgAN. 展开更多

益气活血方治疗气虚血瘀型轻度脊髓型颈椎病的临床研究

【目的】探讨益气活血方(由圣愈汤化裁而来)治疗气虚血瘀型轻度脊髓型颈椎病(CSM)的临床疗效。【方法】将2022年1月至2024年1月就诊于广东省中医院的128例气虚血瘀型轻度CSM患者按随机数字表法随机分为对照组和试验组,每组各64例。对照... 【目的】探讨益气活血方(由圣愈汤化裁而来)治疗气虚血瘀型轻度脊髓型颈椎病(CSM)的临床疗效。【方法】将2022年1月至2024年1月就诊于广东省中医院的128例气虚血瘀型轻度CSM患者按随机数字表法随机分为对照组和试验组,每组各64例。对照组给予口服甲钴胺片治疗,试验组在对照组的基础上联合益气活血方治疗,疗程为4周,并于疗程结束后随访3个月。观察2组患者治疗前、治疗2周后、治疗4周后以及治疗结束3个月的日本骨科学会(JOA)CSM评分和中医证候积分的变化情况,并进行临床疗效评定以及病情进展和不良反应情况评估。【结果】(1)脱落情况方面:研究过程中,对照组脱落2例,试验组脱落3例,最终纳入病例共123例,其中对照组62例,试验组61例。(2)疗效情况方面:治疗结束3个月后,试验组的总有效率为93.44%(57/61),对照组为82.26%(51/62),组间比较(χ^(2)检验),试验组的疗效明显优于对照组,差异有统计学意义(P<0.05)。(3)JOA评分改善情况方面:治疗2周和4周后以及治疗结束3个月,2组患者的JOA评分均较治疗前升高(P<0.05),且试验组在治疗2周和4周后以及治疗结束3个月的JOA评分均高于对照组,差异均有统计学意义(P<0.05)。(4)中医证候积分改善情况方面:治疗2周和4周后以及治疗结束3个月,2组患者的中医证候积分均较治疗前降低(P<0.05),且试验组在治疗2周和4周后以及治疗结束3个月的中医证候积分均低于对照组,差异均有统计学意义(P<0.05)。(5)随访期间,2组患者均未出现病情明显加重或发展至中重度的病例;同时也均未见过敏、胃肠道反应等其他不良事件发生。【结论】益气活血方治疗气虚血瘀型轻度CSM患者临床疗效确切,可以有效改善患者脊髓功能和气虚血瘀证候,减缓患者病情进展,且具有较高的安全性。 展开更多

目的:观察益气活血方联合耳穴压豆治疗气虚血瘀证老年冠心病经皮冠状动脉介入术(PCI)术后患者的临床疗效。方法:将98例2023年3—9月河南中医药大学第一附属医院诊治的气虚血瘀证老年冠心病PCI术后患者,按随机数字表法分为联合组、西医组... 目的:观察益气活血方联合耳穴压豆治疗气虚血瘀证老年冠心病经皮冠状动脉介入术(PCI)术后患者的临床疗效。方法:将98例2023年3—9月河南中医药大学第一附属医院诊治的气虚血瘀证老年冠心病PCI术后患者,按随机数字表法分为联合组、西医组各49例,西医组给予常规治疗,联合组在西医治疗基础上给予益气活血方联合耳穴压豆。治疗4周后,比较2组临床疗效、中医证候评分、心脏功能[左心室射血分数(LVEF)和每分钟心输出量]、运动耐力[6 min步行试验(6MWT)]和西雅图心绞痛(SAQ)量表评分。结果:联合组总有效率为93.88%(46/49),西医组为79.59%(39/49),2组比较,差异有统计学意义(P<0.05)。治疗后,2组各项中医证候评分均较治疗前降低(P<0.05),且联合组各项评分均低于西医组(P<0.05)。治疗后,2组LVEF、每分钟心输出量均较治疗前升高(P<0.05),且联合组LVEF、每分钟心输出量均高于西医组(P<0.05)。治疗后,2组6MWT距离较治疗前延长(P<0.05),且联合组长于西医组(P<0.05)。治疗后,2组躯体受限程度、疾病稳定状态、疾病发作情况、疾病认知程度、疗效满意度评分均较治疗前升高(P<0.05),且联合组SAQ量表各维度评分均高于西医组(P<0.05)。结论:益气活血方联合耳穴压豆治疗冠心病PCI术后气虚血瘀证疗效肯定,可明显改善患者中医证候,增强术后心脏功能及运动耐力,提高患者生活质量。 展开更多

[Methods] The literatures were retrieved from China Knowledge Infrastructure( CNKI),Chinese science and technology journal full-text database( VIP database,VIP),Wanfang Data,the Chinese biomedical Database,Pub Med,Embase,and the Cochrane Library. The retrieval time was set to the creation of the database to January 2017. The randomized controlled trial( RCT) was conducted on the comparison between Yiqi Huoxue Chinese drugs combined with western medicine and the western medicine alone in the treatment of CHD after coronary revascularization. The literature information was extracted and the methodological quality of the included literature was evaluated according to the bias risk assessment tool developed by the Cochrane Collaboration. Meta-analysis was performed with the aid of Rev Man 5. 3 software. Coronary artery restenosis rate,angina pectoris treatment efficiency,left ventricular ejection fraction( LVEF),left ventricular end-diastolic volume( LVEDV),B-type natriuretic peptide( BNP),6-minute walk test( 6 MWT),and adverse reactions were analyzed. [Results] A total of 29 articles were included,a total of2 518 patients,the literature quality was low. OBJECTIVE To explore the curative effect and mechanism of Yiqi Huoxue decoction in the treatment of coronary heart disease with Qi deficiency and blood stasis syndrome.METHODS The patients with coronary heart disease of Qi deficiency and blood stasis syndrome were treated with Yiqi Huoxue decoction for 3 months,and the changes of cardiac function were observed.61 serum samples(including 29 cases of disease group and 32 cases of Yiqi Huoxue expression group)were analyzed by non labeled proteomics.The disease group was used as the control group,and the protein with expression level difference of more than 1.2 folds(P<0.05)was screened.The molecular function,biological pathway and protein interaction of the different proteins were analyzed by bioinformatics,so as to identify the molecular and biological pathway of Yiqi Huoxue decoction in the treatment of coronary heart disease with Qi deficiency and blood stasis syndrome.RESULTS Clinical treatment found that Yiqi Huoxue decoction can improve TCM syndrome score and left ventricular ejection fraction,regulate blood glucose and blood lipid levels,prolong thrombin time,and improve heart function.The results of proteomic quantitative analysis showed that there were 69 proteins with different expression levels in the disease group.Bioinformatics analysis results showed that Yiqi Huoxue decoction may regulate ApoA1,alpha-2 and other proteins to act on HDL assembly,platelet degradation,PI3K Akt signaling pathway,and then play a therapeutic role in coronary heart disease with Qi deficiency and blood stasis syndrome.CONCLUSION Yiqi Huoxue decoction can effectively improved the heart function decline caused by Qi deficiency and blood stasis syndrome of coronary heart disease.It mainly act on energy metabolism and platelet activation pathway by activating HDL assembly and platelet degradation signal pathway proteins.This study can provide reference for the follow-up treatment mechanism of Qi deficiency and blood stasis syndrome of coronary heart disease. 展开更多

目的探讨益气活血通络方对实验性大鼠门静脉血栓(PVT)再通的效果。方法将45只大鼠随机分为实验组40只和对照组5只,采取门静脉间断结扎联合钳夹法建立PVT模型,超声检查判断建模状态。将模型动物随机分为模型组6只、模型对照组6只、利伐... 目的探讨益气活血通络方对实验性大鼠门静脉血栓(PVT)再通的效果。方法将45只大鼠随机分为实验组40只和对照组5只,采取门静脉间断结扎联合钳夹法建立PVT模型,超声检查判断建模状态。将模型动物随机分为模型组6只、模型对照组6只、利伐沙班干预组12只和中药干预组12只。结果超声检查显示PVT模型制备成功;在药物干预2 w后,利伐沙班组再通率为44.4%(4/9),中药干预组为90.9%(10/11,P<0.05);病理学上,模型组、模型对照组、利伐沙班干预组门静脉表现为血管内膜损伤,中膜水肿增厚,大量胶原纤维附着,而中药干预组血管内膜基本完整,中膜略增厚,仅有少量的胶原纤维附着;中药干预组血清D-二聚体水平为(170.7±6.7)ng/mL,利伐沙班处理组为(176.8±7.3)ng/mL,显著低于模型组[(211.3±12.4)ng/mL,P<0.05]或模型对照组[(205.1±12.7)ng/mL,P<0.05]。结论采用间断结扎联合钳夹法成功建立稳定的大鼠PVT模型,益气活血通络方和利伐沙班均可处理使PVT再通,值得深入探讨。 展开更多

目的:观察益气活血化浊解毒方对脑缺血再灌注损伤模型大鼠自噬相关蛋白Beclin-1、LC3-Ⅱ、mTOR表达的影响,初步探讨其作用机制。方法:将60只SPF级SD大鼠随机分为假手术组、模型组、尼莫地平组、中药低剂量组、中药中剂量组、中药高剂量... 目的:观察益气活血化浊解毒方对脑缺血再灌注损伤模型大鼠自噬相关蛋白Beclin-1、LC3-Ⅱ、mTOR表达的影响,初步探讨其作用机制。方法:将60只SPF级SD大鼠随机分为假手术组、模型组、尼莫地平组、中药低剂量组、中药中剂量组、中药高剂量组,每组10只。除假手术组外,其余各组按Zea-Longa线栓法制备大脑中动脉闭塞(MCAO)大鼠模型,造模成功后,各组大鼠分别予相应药物干预,给药3天后进行大鼠神经功能缺损评分;盐酸2,3,5-三苯基四氮(TTC)染色观察脑梗死范围;Nissl染色观察脑组织病理改变;Western blot检测大鼠脑组织mTOR、Beclin-1、LC3-Ⅱ蛋白相对表达量;qRT-PCR检测大鼠脑组织p-mTOR、Beclin-1、LC3-ⅡmRNA表达水平。结果:(1)与假手术组相比,模型组大鼠神经功能缺损评分增加(P<0.05);脑梗死体积百分比增大(P<0.05);细胞病理形态改变严重;Western blot检测结果显示Beclin-1、LC3-Ⅱ蛋白表达水平增多(P<0.05),mTOR蛋白表达水平降低(P<0.05)。qRT-PCR检测结果显示Beclin-1、LC3-ⅡmRNA表达水平均升高(P<0.05),p-mTOR mRNA表达水平降低(P<0.05)。(2)与模型组相比,各治疗组神经功能缺损评分均不同程度下降(P<0.05);脑梗死体积百分比均减少(P<0.05);细胞病理形态呈不同程度的改善,以中药高剂量组与尼莫地平组改善最为明显;Western blot检测结果显示Beclin-1、LC3-Ⅱ蛋白表达水平均降低(P<0.05),mTOR蛋白表达量均升高(P<0.05);qRT-PCR检测结果显示Beclin-1、LC3-ⅡmRNA表达水平均降低(P<0.05),p-mTOR mRNA表达水平均升高(P<0.05)。结论:益气活血化浊解毒方对脑缺血再灌注损伤大鼠缺血脑组织有一定改善作用,可能通过调节Beclin-1、LC3-Ⅱ、mTOR表达,抑制细胞自噬来实现。 展开更多

Objective:To explore the possible mechanism of Huoxue Yiqi recipe promoting angiogenesis by targeting VEGF with miR-484.Methods:The model of miR-484 overexpressed in HUVEC was prepared and intervened with Huoxue Yiqi Recipe Freeze-dried powder.The effects of Huoxue Yiqi Recipe on proliferation,migration and tubular forming ability of HUVEC were separatly detected by CCK-8 method,cell scratch test and Matrigel three-dimensional forming test in vitro.The effect of Huoxue Yiqi Recipe on VEGFA mRNA expression was detected by real-time fluorescence quantitative PCR.Results:Compared with the control group,the proliferation and migration area of cells in miR-484 overexpression model group decreased,the length,number of branches and nodes involved in tube formation decreased,the tube formation ability decreased significantly,and the expression level of VEGFA mRNA decreased,with statistical significance.Compared with the model group,the cell proliferation ability,migration area,length,number of branches and nodes involved in tube formation,tube formation ability and expression level of VEGFA mRNA in the Huoxue Yiqi Prescription group were up-regulated,and the difference was statistically significant(P<0.05).Conclusion:MiR-484 inhibits the expression of VEGFA and the proliferation,migration and formation of HUVEC,and Huoxue Yiqi recipe has a certain negative regulation effect on this process,which may be a new target of Huoxue Yiqi recipe to promote angiogenesis. 展开更多
