Nutech functional score: A novel scoring system to assess spinal cord injury patients

AIM To develop a new scoring system, nutech functional scores(NFS) for assessing the patients with spinal cord injury(SCI).METHODS The conventional scale, American Spinal Injury Association's(ASIA) impairment scale is a measure which precisely describes the severity of the SCI.However, it has various limitations which lead to incomplete assessment of SCI patients.We have developed a 63 point scoring system, i.e., NFS for patients suffering with SCI.A list of symptoms either common or rare that were found to be associated with SCI was recorded for each patient.On the basis of these lists, we have developed NFS.RESULTS These lists served as a base to prepare NFS, a 63 point positional(each symptom is sub-graded and get points based on position) and directional(moves in direction BAD → GOOD) scoring system.For non-progressive diseases, 1, 2, 3, 4, 5 denote worst, bad, moderate, good and best(normal), respectively.NFS for SCI has been divided into different groups based on the affected part of the body being assessed, i.e., motor assessment(shoulders, elbow, wrist, fingers-grasp, fingers-release, hip, knee, ankle and toe), sensory assessment, autonomic assessment, bed sore assessment and general assessment.As probability based studies required a range of(-1, 1) or at least the range of(0, 1) to be useful for real world analysis, the grades were converted to respective numeric values.CONCLUSION NFS can be considered as a unique tool to assess the improvement in patients with SCI as it overcomes the limitations of ASIA impairment scale. 展开更多

Correlation analyse between thyroid hormone levels and severity of schizophrenia symptoms

BACKGROUND The imbalance of hormone levels in the body is closely related to the occurrence and progression of schizophrenia,especially thyroid hormones.AIM To study the relationship between triiodothyronine(T3),thyroxine(T4),free T3(FT3),free T4(FT4),thyroid stimulating hormone(TSH)and schizophrenia.METHODS In this study,100 schizophrenia patients were selected from our hospital between April 2022 and April 2024.Their clinical data were analyzed retrospectively.Based on the Positive and Negative Syndrome Scale(PANSS)score,patients were divided into mild(1-3 points,n=39),moderate(4 points,n=45),and severe groups(5-7 points,n=16).Additionally,55 healthy individuals served as a control group.Venous blood samples were collected to measure T3,T4,FT3,FT4,TSH,and cortisol concentrations,analyzing their relationship with PANSS scores.RESULTS The serum levels of T3,FT3,FT4,TSH and cortisol in the schizophrenia group were lower than those in the control group(P<0.05).With the increase of the severity of the disease,the concentrations of T3 and T4 decreased,while the con-centrations of TSH and cortisol increased(P<0.05).The concentrations of TSH and cortisol were positively correlated with the PANSS score,while T3 and T4 were negatively correlated with the PANSS score(P<0.05).The receiver ope-rating characteristic curve results showed that T3,T4,TSH,and cortisol had good efficacy in the diagnosis of schizophrenia.Logistic results showed that decreased T3 level,decreased T4 level,decreased TSH level and increased cortisol level may be independent risk factors for schizophrenia.CONCLUSION Thyroid hormone levels are associated with the severity of schizophrenia symptoms,which can provide new solutions for the diagnosis and treatment of schizophrenia. 展开更多

Correlation between imaging features on computed tomography and combined positive score of PD-L1 expression in patients with gastric cancer

Objective:To explore the correlation between computed tomography(CT)features and combined positive score(CPS)of programmed cell death ligand 1(PD-L1)expression in patients with gastric cancer(GC).Methods:This study reviewed an institutional database of patients who underwent GC operation without neoadjuvant chemotherapy between December 2019 and September 2020.The CPS results of PD-L1 expression of postoperative histological examination were recorded by pathology.Baseline CT features were measured,and their correlation with CPS 5 or 10 score groups of PD-L1 expression was analyzed.Results:Data for 153 patients with GC were collected.Among them,124 were advanced GC patients,and 29were early GC patients.None of the CT features significantly differed between CPS groups with a cutoff score of 5and a score of 10 in patients with early GC.In advanced GC,the presence of lymph nodes with short diameters>10mm was significantly different(P=0.024)between the CPS<5 and CPS≥5 groups.CT features such as tumor attenuation in the arterial phase,long and short diameter of the largest lymph node,the sum of long diameter of the two largest lymph nodes,the sum of short diameter of the two largest lymph nodes,and the presence of lymph nodes with short diameters>10 mm significantly differed between the CPS<10 and CPS≥10 groups in advanced GC.The sensitivity,specificity and area under receiver operating characteristic(ROC)curve of logistic regression model for predicting CPS≥10 was 71.7%,50.0%and 0.671,respectively.Microsatellite instability(MSI)status was significantly different in CPS groups with cutoff score of 5 and 10 in advanced GC patients.Conclusions:CT findings of advanced GC patients with CPS≥10 showed greater arterial phase enhancement and larger lymph nodes.CT has the potential to help screen patients suitable for immunotherapy. 展开更多

The identification of hepatitis C virus(HCV)virus-human protein interactions will not only help us understand the molecular mechanisms of related diseases but also be conductive to discovering new drug targets.An increasing number of clinically and experimentally validated interactions between HCV and human proteins have been documented in public databases,facilitating studies based on computational methods.In this study,we proposed a new computational approach,rotation forest position-specific scoring matrix(RF-PSSM),to predict the interactions among HCV and human proteins.In particular,PSSM was used to characterize each protein,two-dimensional principal component analysis(2DPCA)was then adopted for feature extraction of PSSM.Finally,rotation forest(RF)was used to implement classification.The results of various ablation experiments show that on independent datasets,the accuracy and area under curve(AUC)value of RF-PSSM can reach 93.74% and 94.29%,respectively,outperforming almost all cutting-edge research.In addition,we used RF-PSSM to predict 9 human proteins that may interact with HCV protein E1,which can provide theoretical guidance for future experimental studies. 展开更多
