The time delay part of the model is calculated by a recursive least squares algorithm of system identification, which can gradually forget past information. The grey series part of the model uses an equal dimension new information model (EDNIM) and it applies 3 points smoothing method to preprocess the original data and modify remnant difference by GM(1,1). Through the optimization of the coefficient of the model, we are able to minimize the error variance of predictive data. A case study shows that the proposed method achieved high calculation precision and speed and it can be used to obtain the predictive value in real time state estimation of power distribution networks.展开更多

After the structure and mathematical description being given, dynamic back propagation (BP) algorithm of training weights of Elman neural network is deduced. At last, the network is used to predict ash content of black amber in jigging production process. BACKGROUND Integrating conventional ultrasound features with 2D shear wave elastography(2D-SWE)can potentially enhance preoperative hepatocellular carcinoma(HCC)predictions.AIM To develop a 2D-SWE-based predictive mod...BACKGROUND Integrating conventional ultrasound features with 2D shear wave elastography(2D-SWE)can potentially enhance preoperative hepatocellular carcinoma(HCC)predictions.AIM To develop a 2D-SWE-based predictive model for preoperative identification of HCC.METHODS A retrospective analysis of 884 patients who underwent liver resection and pathology evaluation from February 2021 to August 2023 was conducted at the Oriental Hepatobiliary Surgery Hospital.The patients were divided into the modeling group(n=720)and the control group(n=164).The study included conventional ultrasound,2D-SWE,and preoperative laboratory tests.Multiple logistic regression was used to identify independent predictive factors for RESULTS In the modeling group analysis,maximal elasticity(Emax)of tumors and their peripheries,platelet count,cirrhosis,and blood flow were independent risk indicators for malignancies.These factors yielded an area under the curve of 0.77(95%confidence interval:0.73-0.81)with 84%sensitivity and 61%specificity.The model demonstrated good calibration in both the construction and validation cohorts,as shown by the calibration graph and Hosmer-Lemeshow test(P=0.683 and P=0.658,respectively).Additionally,the mean elasticity(Emean)of the tumor periphery was identified as a risk factor for microvascular invasion(MVI)in malignant liver tumors(P=0.003).Patients receiving antiviral treatment differed significantly in platelet count(P=0.002),Emax of tumors(P=0.033),Emean of tumors(P=0.042),Emax at tumor periphery(P<0.001),and Emean at tumor periphery(P=0.003).CONCLUSION 2D-SWE’s hardness value serves as a valuable marker for enhancing the preoperative diagnosis of malignant liver lesions,correlating significantly with MVI and antiviral treatment efficacy.展开更多
BACKGROUND Transjugular intrahepatic portosystemic shunt(TIPS)placement is a procedure that can effectively treat complications of portal hypertension,such as variceal bleeding and refractory ascites.However,there hav...BACKGROUND Transjugular intrahepatic portosystemic shunt(TIPS)placement is a procedure that can effectively treat complications of portal hypertension,such as variceal bleeding and refractory ascites.However,there have been no specific studies on predicting long-term survival after TIPS placement.AIM To establish a model to predict long-term survival in patients with hepatitis cirrhosis after TIPS.METHODS A retrospective analysis was conducted on a cohort of 224 patients who un-derwent TIPS implantation.Through univariate and multivariate Cox regression analyses,various factors were examined for their ability to predict survival at 6 years after TIPS.Consequently,a composite score was formulated,encompassing the indication,shunt reasonability,portal venous pressure gradient(PPG)after TIPS,percentage decrease in portal venous pressure(PVP),indocyanine green retention rate at 15 min(ICGR15)and total bilirubin(Tbil)level.Furthermore,the performance of the newly developed Cox(NDC)model was evaluated in an in-ternal validation cohort and compared with that of a series of existing models.RESULTS The indication(variceal bleeding or ascites),shunt reasonability(reasonable or unreasonable),ICGR15,post-operative PPG,percentage of PVP decrease and Tbil were found to be independent factors affecting long-term survival after TIPS placement.The NDC model incorporated these parameters and successfully identified patients at high risk,exhibiting a notably elevated mortality rate following the TIPS procedure,as observed in both the training and validation cohorts.Additionally,in terms of predicting the long-term survival rate,the performance of the NDC model was significantly better than that of the other four models[Child-Pugh,model for end-stage liver disease(MELD),MELD-sodium and the Freiburg index of post-TIPS survival].CONCLUSION The NDC model can accurately predict long-term survival after the TIPS procedure in patients with hepatitis cirrhosis,help identify high-risk patients and guide follow-up management after TIPS implantation.展开更多

Objective:To construct a risk prediction model for fall in patients with maintenance hemodialysis(MHD)and to verify the prediction effect of the model.Methods:From June 2020 to December 2020,307 patients who underwent...Objective:To construct a risk prediction model for fall in patients with maintenance hemodialysis(MHD)and to verify the prediction effect of the model.Methods:From June 2020 to December 2020,307 patients who underwent MHD in a tertiary hospital in Chengdu were divided into a fall group(32 cases)and a non-fall group(275 cases).Logistic regression analysis model was used to establish the influencing factors of the subjects.Hosmer–Lemeshow and receiver operating characteristic(ROC)curve were used to test the goodness of fit and predictive effect of the model,and 104 patients were again included in the application research of the model.Results:The risk factors for fall were history of falls in the past year(OR=3.951),dialysis-related hypotension(OR=6.949),time up and go(TUG)test(OR=4.630),serum albumin(OR=0.661),frailty(OR=7.770),and fasting blood glucose(OR=1.141).Hosmer–Lemeshow test was P=0.475;the area under the ROC curve was 0.907;the Youden index was 0.642;the sensitivity was 0.843;and the specificity was 0.799.Conclusions:The risk prediction model constructed in this study has a good effect and can provide references for clinical screening of fall risks in patients with MHD.展开更多
BACKGROUND Colorectal cancer(CRC)is one of the most prevalent and lethal malignant tumors worldwide.Currently,surgical intervention was the primary treatment modality for CRC.However,increasing studies have revealed t...BACKGROUND Colorectal cancer(CRC)is one of the most prevalent and lethal malignant tumors worldwide.Currently,surgical intervention was the primary treatment modality for CRC.However,increasing studies have revealed that CRC patients may experience postoperative cognitive dysfunction(POCD).AIM To establish a risk prediction model for POCD in CRC patients and investigate the preventive value of dexmedetomidine(DEX).METHODS A retrospective analysis was conducted on clinical data from 140 CRC patients who underwent surgery at the People’s Hospital of Qian Nan from February 2020 to May 2024.Patients were allocated into a modeling group(n=98)and a validation group(n=42)in a 7:3 ratio.General clinical data were collected.Additionally,in the modeling group,patients who received DEX preoperatively were incorporated into the observation group(n=54),while those who did not were placed in the control group(n=44).The incidence of POCD was recorded for both cohorts.Data analysis was performed using statistical product and service solutions 20.0,with t-tests orχ2 tests employed for group comparisons based on the data type.Least absolute shrinkage and selection operator regression was applied to identify influencing factors and reduce the impact of multicollinear predictors among variables.Multivariate analysis was carried out using Logistic regression.Based on the identified risk factors,a risk prediction model for POCD in CRC patients was developed,and the predictive value of these risk factors was evaluated.RESULTS Significant differences were observed between the cognitive dysfunction group and the non-cognitive dysfunction group in diabetes status,alcohol consumption,years of education,anesthesia duration,intraoperative blood loss,intraoperative hypoxemia,use of DEX during surgery,intraoperative use of vasoactive drugs,surgical time,systemic inflammatory response syndrome(SIRS)score(P<0.05).Multivariate Logistic regression analysis identified that diabetes[odds ratio(OR)=4.679,95%confidence interval(CI)=1.382-15.833],alcohol consumption(OR=5.058,95%CI:1.255-20.380),intraoperative hypoxemia(OR=4.697,95%CI:1.380-15.991),no use of DEX during surgery(OR=3.931,95%CI:1.383-11.175),surgery duration≥90 minutes(OR=4.894,95%CI:1.377-17.394),and a SIRS score≥3(OR=4.133,95%CI:1.323-12.907)were independent risk factors for POCD in CRC patients(P<0.05).A risk prediction model for POCD was constructed using diabetes,alcohol consumption,intraoperative hypoxemia,non-use of DEX during surgery,surgery duration,and SIRS score as factors.A receiver operator characteristic curve analysis of these factors revealed the model’s predictive sensitivity(88.56%),specificity(70.64%),and area under the curve(AUC)(AUC=0.852,95%CI:0.773-0.919).The model was validated using 42 CRC patients who met the inclusion criteria,demonstrating sensitivity(80.77%),specificity(81.25%),and accuracy(80.95%),and AUC(0.805)in diagnosing cognitive impairment,with a 95%CI:0.635-0.896.CONCLUSION Logistic regression analysis identified that diabetes,alcohol consumption,intraoperative hypoxemia,non-use of DEX during surgery,surgery duration,and SIRS score vigorously influenced the occurrence of POCD.The risk prediction model based on these factors demonstrated good predictive performance for POCD in CRC individuals.This study offers valuable insights for clinical practice and contributes to the prevention and management of POCD under CRC circumstances.展开更多

This study compares the performance of Holt-Winters’ Exponential Smoothing, Harmonic, and Artificial Neural Network (ANN) models using data from January 2018 to December 2023, incorporating both historical case records from Sierra Leone’s Health Management Information System (HMIS) and meteorological variables including humidity, precipitation, and temperature. The ANN model demonstrated superior performance, achieving a Mean Absolute Percentage Error (MAPE) of 4.74% before including climatic variables. This was further reduced to 3.9% with the inclusion of climatic variables, outperforming traditional models like Holt-Winters and Harmonic, which yielded MAPEs of 22.53% and 17.90% respectively. The ANN’s success is attributed to its ability to capture complex, non-linear relationships in the data, particularly when enhanced with relevant climatic variables. Using the optimized ANN model, we forecasted malaria cases for the next 24 months, predicting a steady increase from January 2024 to December 2025, with seasonal peaks. This study underscores the potential of machine learning approaches, particularly ANNs, in epidemiological modelling and highlights the importance of integrating environmental factors into malaria prediction models, recommending the ANN model for informing more targeted and efficient malaria control strategies to improve public health outcomes in Sierra Leone and similar settings.展开更多

The primary goal is to evaluate the effectiveness of these models in predicting healthcare outcomes using patient records, specifically the Cancerpatient.xlsx dataset, which tracks variables such as patient age, symptoms, genetic risk factors, and environmental exposures over time. The proposed strategy involves training each model on historical patient data to predict age progression and other related health indicators, with performance evaluated using Mean Squared Error (MSE) and Root Mean Squared Error (RMSE) metrics. Our findings reveal that neural networks consistently outperform ARIMA and SARIMA by capturing non-linear patterns and complex temporal dependencies within the dataset, resulting in lower forecasting errors. BACKGROUND Breast cancer(BC)is the second most common malignancy globally.Young and middle-aged patients face more pressures from diagnosis,treatment,costs,and psychological issues like self-image concerns,social barr...BACKGROUND Breast cancer(BC)is the second most common malignancy globally.Young and middle-aged patients face more pressures from diagnosis,treatment,costs,and psychological issues like self-image concerns,social barriers,and professional challenges.Compared to other age groups,they have higher recurrence rates,lower survival rates,and increased risk of depression.Research is lacking on factors influencing depressive symptoms and predictive models for this age group.AIM To analyze factors influencing depressive symptoms in young/middle-aged BC patients and construct a depression risk predictive model.METHODS A total of 360 patients undergoing BC treatment at two tertiary hospitals in Jiangsu Province,China from November 2023 to April 2024 were included in the study.Participants were surveyed using a general information questionnaire,the patient health questionnaire depression scale,the visual analog scale for pain,the revised family support scale,and the long form of the international physical activity questionnaire.Univariate and multivariate analyses were conducted to identify the factors affecting depression in middle-aged and young BC patients,and a predictive model for depression risk was developed based on these findings.RESULTS Among the 360 middle-aged and young BC patients,the incidence rate of depressive symptoms was 38.61%(139/360).Multivariate analysis revealed that tumor grade,patient’s monthly income,pain score,family support score,and physical activity score were factors influencing depression in this patient group(P<0.05).The risk prediction model constructed based on these factors yielded an area under the receiver operating characteristic curve of 0.852,with a maximum Youden index of 0.973,sensitivity of 86.80%,specificity of 89.50%,and a diagnostic odds ratio of 0.552.The Hosmer-Lemeshow test for goodness of fit indicated an adequate model fit(χ^(2)=0.360,P=0.981).CONCLUSION The constructed predictive model demonstrates good predictive performance and can serve as a reference for medical professionals to early identify high-risk patients and implement corresponding preventive measures to decrease the incidence of depressive symptoms in this population.展开更多

BACKGROUND The risk factors and prediction models for diabetic foot(DF)remain incompletely understood,with several potential factors still requiring in-depth investigations.AIM To identify risk factors for new-onset D...BACKGROUND The risk factors and prediction models for diabetic foot(DF)remain incompletely understood,with several potential factors still requiring in-depth investigations.AIM To identify risk factors for new-onset DF and develop a robust prediction model for hospitalized patients with type 2 diabetes.METHODS We included 6301 hospitalized patients with type 2 diabetes from January 2016 to December 2021.A univariate Cox model and least absolute shrinkage and selection operator analyses were applied to select the appropriate predictors.Nonlinear associations between continuous variables and the risk of DF were explored using restricted cubic spline functions.The Cox model was further employed to evaluate the impact of risk factors on DF.The area under the curve(AUC)was measured to evaluate the accuracy of the prediction model.RESULTS Seventy-five diabetic inpatients experienced DF.The incidence density of DF was 4.5/1000 person-years.A long duration of diabetes,lower extremity arterial disease,lower serum albumin,fasting plasma glucose(FPG),and diabetic nephropathy were independently associated with DF.Among these risk factors,the serum albumin concentration was inversely associated with DF,with a hazard ratio(HR)and 95%confidence interval(CI)of 0.91(0.88-0.95)(P<0.001).Additionally,a U-shaped nonlinear relationship was observed between the FPG level and DF.After adjusting for other variables,the HRs and 95%CI for FPG<4.4 mmol/L and≥7.0 mmol/L were 3.99(1.55-10.25)(P=0.004)and 3.12(1.66-5.87)(P<0.001),respectively,which was greater than the mid-range level(4.4-6.9 mmol/L).The AUC for predicting DF over 3 years was 0.797.CONCLUSION FPG demonstrated a U-shaped relationship with DF.Serum albumin levels were negatively associated with DF.The prediction nomogram model of DF showed good discrimination ability using diabetes duration,lower extremity arterial disease,serum albumin,FPG,and diabetic nephropathy(Clinicaltrial.gov NCT05519163).展开更多
This study focuses on empirical modeling of the strength characteristics of urban soils contaminated with heavy metals using machine learning tools and their subsequent stabilization with ordinary Portland cement(OPC)...This study focuses on empirical modeling of the strength characteristics of urban soils contaminated with heavy metals using machine learning tools and their subsequent stabilization with ordinary Portland cement(OPC).For dataset collection,an extensive experimental program was designed to estimate the unconfined compressive strength(Qu)of heavy metal-contaminated soils collected from awide range of land use pattern,i.e.residential,industrial and roadside soils.Accordingly,a robust comparison of predictive performances of four data-driven models including extreme learning machines(ELMs),gene expression programming(GEP),random forests(RFs),and multiple linear regression(MLR)has been presented.For completeness,a comprehensive experimental database has been established and partitioned into 80%for training and 20%for testing the developed models.Inputs included varying levels of heavy metals like Cd,Cu,Cr,Pb and Zn,along with OPC.The results revealed that the GEP model outperformed its counterparts:explaining approximately 96%of the variability in both training(R2=0.964)and testing phases(R^(2)=0.961),and thus achieving the lowest RMSE and MAE values.ELM performed commendably but was slightly less accurate than GEP whereas MLR had the lowest performance metrics.GEP also provided the benefit of traceable mathematical equation,enhancing its applicability not just as a predictive but also as an explanatory tool.Despite its insights,the study is limited by its focus on a specific set of heavy metals and urban soil samples of a particular region,which may affect the generalizability of the findings to different contamination profiles or environmental conditions.The study recommends GEP for predicting Qu in heavy metal-contaminated soils,and suggests further research to adapt these models to different environmental conditions.展开更多

BACKGROUND Colorectal polyps are precancerous diseases of colorectal cancer.Early detection and resection of colorectal polyps can effectively reduce the mortality of colorectal cancer.Endoscopic mucosal resection(EMR...BACKGROUND Colorectal polyps are precancerous diseases of colorectal cancer.Early detection and resection of colorectal polyps can effectively reduce the mortality of colorectal cancer.Endoscopic mucosal resection(EMR)is a common polypectomy proce-dure in clinical practice,but it has a high postoperative recurrence rate.Currently,there is no predictive model for the recurrence of colorectal polyps after EMR.AIM To construct and validate a machine learning(ML)model for predicting the risk of colorectal polyp recurrence one year after EMR.METHODS This study retrospectively collected data from 1694 patients at three medical centers in Xuzhou.Additionally,a total of 166 patients were collected to form a prospective validation set.Feature variable screening was conducted using uni-variate and multivariate logistic regression analyses,and five ML algorithms were used to construct the predictive models.The optimal models were evaluated based on different performance metrics.Decision curve analysis(DCA)and SHapley Additive exPlanation(SHAP)analysis were performed to assess clinical applicability and predictor importance.RESULTS Multivariate logistic regression analysis identified 8 independent risk factors for colorectal polyp recurrence one year after EMR(P<0.05).Among the models,eXtreme Gradient Boosting(XGBoost)demonstrated the highest area under the curve(AUC)in the training set,internal validation set,and prospective validation set,with AUCs of 0.909(95%CI:0.89-0.92),0.921(95%CI:0.90-0.94),and 0.963(95%CI:0.94-0.99),respectively.DCA indicated favorable clinical utility for the XGBoost model.SHAP analysis identified smoking history,family history,and age as the top three most important predictors in the model.CONCLUSION The XGBoost model has the best predictive performance and can assist clinicians in providing individualized colonoscopy follow-up recommendations.展开更多
BACKGROUND The discrepancy between endoscopic biopsy pathology and the overall pathology of gastric low-grade intraepithelial neoplasia(LGIN)presents challenges in developing diagnostic and treatment protocols.AIM To ...BACKGROUND The discrepancy between endoscopic biopsy pathology and the overall pathology of gastric low-grade intraepithelial neoplasia(LGIN)presents challenges in developing diagnostic and treatment protocols.AIM To develop a risk prediction model for the pathological upgrading of gastric LGIN to aid clinical diagnosis and treatment.METHODS We retrospectively analyzed data from patients newly diagnosed with gastric LGIN who underwent complete endoscopic resection within 6 months at the First Medical Center of Chinese People’s Liberation Army General Hospital between January 2008 and December 2023.A risk prediction model for the pathological progression of gastric LGIN was constructed and evaluated for accuracy and clinical applicability.RESULTS A total of 171 patients were included in this study:93 patients with high-grade intraepithelial neoplasia or early gastric cancer and 78 with LGIN.The logistic stepwise regression model demonstrated a sensitivity and specificity of 0.868 and 0.800,respectively,while the least absolute shrinkage and selection operator(LASSO)regression model showed sensitivity and specificity values of 0.842 and 0.840,respectively.The area under the curve(AUC)for the logistic model was 0.896,slightly lower than the AUC of 0.904 for the LASSO model.Internal validation with 30%of the data yielded AUC scores of 0.908 for the logistic model and 0.905 for the LASSO model.The LASSO model provided greater utility in clinical decision-making.CONCLUSION A risk prediction model for the pathological upgrading of gastric LGIN based on white-light and magnifying endoscopic features can accurately and effectively guide clinical diagnosis and treatment.展开更多
BACKGROUND Patients with chronic obstructive pulmonary disease(COPD)frequently experience exacerbations requiring multiple hospitalizations over prolonged disease courses,which predispose them to generalized anxiety d...BACKGROUND Patients with chronic obstructive pulmonary disease(COPD)frequently experience exacerbations requiring multiple hospitalizations over prolonged disease courses,which predispose them to generalized anxiety disorder(GAD).This comorbidity exacerbates breathing difficulties,activity limitations,and social isolation.While previous studies predominantly employed the GAD 7-item scale for screening,this approach is somewhat subjective.The current literature on predictive models for GAD risk in patients with COPD is limited.AIM To construct and validate a GAD risk prediction model to aid healthcare professionals in preventing the onset of GAD.METHODS This retrospective analysis encompassed patients with COPD treated at our institution from July 2021 to February 2024.The patients were categorized into a modeling(MO)group and a validation(VA)group in a 7:3 ratio on the basis of the occurrence of GAD.Univariate and multivariate logistic regression analyses were utilized to construct the risk prediction model,which was visualized using forest plots.The model’s performance was evaluated using Hosmer-Lemeshow(H-L)goodness-of-fit test and receiver operating characteristic(ROC)curve analysis.RESULTS A total of 271 subjects were included,with 190 in the MO group and 81 in the VA group.GAD was identified in 67 patients with COPD,resulting in a prevalence rate of 24.72%(67/271),with 49 cases(18.08%)in the MO group and 18 cases(22.22%)in the VA group.Significant differences were observed between patients with and without GAD in terms of educational level,average household income,smoking history,smoking index,number of exacerbations in the past year,cardiovascular comorbidities,disease knowledge,and personality traits(P<0.05).Multivariate logistic regression analysis revealed that lower education levels,household income<3000 China yuan,smoking history,smoking index≥400 cigarettes/year,≥two exacerbations in the past year,cardiovascular comorbidities,complete lack of disease information,and introverted personality were significant risk factors for GAD in the MO group(P<0.05).ROC analysis indicated that the area under the curve for predicting GAD in the MO and VA groups was 0.978 and 0.960.The H-L test yieldedχ^(2) values of 6.511 and 5.179,with P=0.275 and 0.274.Calibration curves demonstrated good agreement between predicted and actual GAD occurrence risks.CONCLUSION The developed predictive model includes eight independent risk factors:Educational level,household income,smoking history,smoking index,number of exacerbations in the past year,presence of cardiovascular comorbidities,level of disease knowledge,and personality traits.This model effectively predicts the onset of GAD in patients with COPD,enabling early identification of high-risk individuals and providing a basis for early preventive interventions by nursing staff.展开更多

This study aims to develop and validate a clinical prediction model for postoperative recurrence in pancreatic cancer patients, incorporating multiple preoperative, intraoperative, and postoperative factors to assist clinical decision-making. Methods: A retrospective study was conducted on 216 patients who underwent surgical treatment for pancreatic malignancy at the First Affiliated Hospital of Chongqing Medical University between January 2015 and January 2023. An independent external validation cohort of 76 patients from the Second Affiliated Hospital of Chongqing Medical University was used to validate the model. Seven independent risk factors for postoperative recurrence were identified through univariate and multivariate Cox regression analyses. The model’s performance was evaluated using the concordance index (C-index) and ROC curves, and its accuracy and clinical value were assessed using calibration curves and decision curve analysis (DCA). Results: The predictive model demonstrated good discriminatory power, with a C-index of 0.72 in the training cohort and 0.66 in the validation cohort. The ROC curves for predicting recurrence at 3, 6, and 12 months postoperatively showed AUC values ranging from 0.72 to 0.83, indicating strong predictive value. Calibration curves and DCA confirmed the model’s accuracy and clinical utility. Conclusion: This study successfully developed and validated a clinical prediction model that incorporates seven independent risk factors for postoperative recurrence in pancreatic cancer. The model provides a useful tool for predicting recurrence risk, aiding in the identification of high-risk patients, and informing clinical decision-making.展开更多
Objective:To evaluate the use of infrared thermography technology for objective and quantitative syndrome differentiation and treatment in traditional Chinese medicine(TCM),specifically in patients with Chaihu Guizhi ...Objective:To evaluate the use of infrared thermography technology for objective and quantitative syndrome differentiation and treatment in traditional Chinese medicine(TCM),specifically in patients with Chaihu Guizhi Ganjiang Decoction syndrome.Methods:Data were collected from over 100 patients diagnosed with Chaihu Guizhi Ganjiang Decoction syndrome at Professor Li Leyu’s endocrinology clinic,Zhongshan Hospital of Traditional Chinese Medicine,Guangdong Province,between April 2021 and April 2022.Body surface temperature data were obtained using the MTI-EXPRO-2013-B infrared thermography system.Principal component analysis(PCA)was applied to differentiate temperature distribution characteristics between genders,and a neural network prediction model was constructed for syndrome diagnosis.Results:Infrared thermography effectively captured surface temperature characteristics of patients with Chaihu Guizhi Ganjiang Decoction syndrome.PCA identified one principal component with a variance explanation rate of 73.953%for females and two principal components with a cumulative variance explanation rate of 77.627%for males.The neural network model demonstrated high predictive performance,with an area under the ROC curve of 0.9743 for the training set and 0.9889 for the validation set.Sensitivity was 1,specificity 0.8636,precision 0.8846,accuracy 0.9333,and the F1 score 0.9388.Conclusion:Infrared thermography provides an innovative,objective,and quantitative method for syndrome differentiation and treatment in TCM.It represents a significant advancement in transitioning from traditional empirical approaches to modern,visualized,and precise diagnosis and treatment.This study underscores the potential of integrating advanced technologies in TCM for enhanced clinical application and modernization.展开更多
BACKGROUND:Intracranial hemorrhage (ICH),a severe complication among adults receiving extracorporeal membrane oxygenation (ECMO),is often related to poor outcomes.This study aimed to establish a predictive model for I...BACKGROUND:Intracranial hemorrhage (ICH),a severe complication among adults receiving extracorporeal membrane oxygenation (ECMO),is often related to poor outcomes.This study aimed to establish a predictive model for ICH in adults receiving ECMO treatment.METHODS:Adults who received ECMO between January 2017 and June 2022 were the subjects of a single-center retrospective study.Patients under the age of 18 years old,with acute ICH before ECMO,with less than 24 h of ECMO support,and with incomplete data were excluded.ICH was diagnosed by a head computed tomography scan.The outcomes included the incidence of ICH,in-hosptial mortality and 28-day mortality.Multivariate logistic regression analysis was used to identify relevant risk factors of ICH,and a predictive model of ICH with a nomogram was constructed.RESULTS:Among the 227 patients included,22 developed ICH during ECMO.Patients with ICH had higher in-hospital mortality (90.9%vs.47.8%,P=0.001) and higher 28-day mortality (81.8%vs.47.3%,P=0.001) than patients with non-ICH.ICH was associated with decreased grey-white-matter ratio (GWR)(OR=0.894,95%CI:0.841–0.951,P<0.001),stroke history (OR=4.265,95%CI:1.052–17.291,P=0.042),fresh frozen plasma (FFP) transfusion (OR=1.208,95%CI:1.037–1.408,P=0.015)and minimum platelet (PLT) count during ECMO support (OR=0.977,95%CI:0.958–0.996,P=0.019).The area under the receiver operating characteristic curve of the ICH predictive model was 0.843 (95%CI:0.762–0.924,P<0.001).CONCLUSION:ECMO-treated patients with ICH had a higher risk of death.GWR,stroke history,FFP transfusion,and the minimum PLT count were independently associated with ICH,and the ICH predictive model showed that these parameters performed well as diagnostic tools.展开更多
文摘A new combined model is proposed to obtain predictive data value applied in state estimation for radial power distribution networks. The time delay part of the model is calculated by a recursive least squares algorithm of system identification, which can gradually forget past information. The grey series part of the model uses an equal dimension new information model (EDNIM) and it applies 3 points smoothing method to preprocess the original data and modify remnant difference by GM(1,1). Through the optimization of the coefficient of the model, we are able to minimize the error variance of predictive data. A case study shows that the proposed method achieved high calculation precision and speed and it can be used to obtain the predictive value in real time state estimation of power distribution networks.
文摘Based on the comparison of several methods of time series predicting, this paper points out that it is necessary to use dynamic neural network in modeling of complex production process. Because self feedback and mutual feedback are adopted among nodes at the same layer in Elman network, it has stronger ability of dynamic approximation, and can describe any non linear dynamic system. After the structure and mathematical description being given, dynamic back propagation (BP) algorithm of training weights of Elman neural network is deduced. At last, the network is used to predict ash content of black amber in jigging production process. The results show that this neural network is powerful in predicting and suitable for modeling, predicting, and controling of complex production process.

文摘BACKGROUND Integrating conventional ultrasound features with 2D shear wave elastography(2D-SWE)can potentially enhance preoperative hepatocellular carcinoma(HCC)predictions.AIM To develop a 2D-SWE-based predictive model for preoperative identification of HCC.METHODS A retrospective analysis of 884 patients who underwent liver resection and pathology evaluation from February 2021 to August 2023 was conducted at the Oriental Hepatobiliary Surgery Hospital.The patients were divided into the modeling group(n=720)and the control group(n=164).The study included conventional ultrasound,2D-SWE,and preoperative laboratory tests.Multiple logistic regression was used to identify independent predictive factors for RESULTS In the modeling group analysis,maximal elasticity(Emax)of tumors and their peripheries,platelet count,cirrhosis,and blood flow were independent risk indicators for malignancies.These factors yielded an area under the curve of 0.77(95%confidence interval:0.73-0.81)with 84%sensitivity and 61%specificity.The model demonstrated good calibration in both the construction and validation cohorts,as shown by the calibration graph and Hosmer-Lemeshow test(P=0.683 and P=0.658,respectively).Additionally,the mean elasticity(Emean)of the tumor periphery was identified as a risk factor for microvascular invasion(MVI)in malignant liver tumors(P=0.003).Patients receiving antiviral treatment differed significantly in platelet count(P=0.002),Emax of tumors(P=0.033),Emean of tumors(P=0.042),Emax at tumor periphery(P<0.001),and Emean at tumor periphery(P=0.003).CONCLUSION 2D-SWE’s hardness value serves as a valuable marker for enhancing the preoperative diagnosis of malignant liver lesions,correlating significantly with MVI and antiviral treatment efficacy.

文摘BACKGROUND Transjugular intrahepatic portosystemic shunt(TIPS)placement is a procedure that can effectively treat complications of portal hypertension,such as variceal bleeding and refractory ascites.However,there have been no specific studies on predicting long-term survival after TIPS placement.AIM To establish a model to predict long-term survival in patients with hepatitis cirrhosis after TIPS.METHODS A retrospective analysis was conducted on a cohort of 224 patients who un-derwent TIPS implantation.Through univariate and multivariate Cox regression analyses,various factors were examined for their ability to predict survival at 6 years after TIPS.Consequently,a composite score was formulated,encompassing the indication,shunt reasonability,portal venous pressure gradient(PPG)after TIPS,percentage decrease in portal venous pressure(PVP),indocyanine green retention rate at 15 min(ICGR15)and total bilirubin(Tbil)level.Furthermore,the performance of the newly developed Cox(NDC)model was evaluated in an in-ternal validation cohort and compared with that of a series of existing models.RESULTS The indication(variceal bleeding or ascites),shunt reasonability(reasonable or unreasonable),ICGR15,post-operative PPG,percentage of PVP decrease and Tbil were found to be independent factors affecting long-term survival after TIPS placement.The NDC model incorporated these parameters and successfully identified patients at high risk,exhibiting a notably elevated mortality rate following the TIPS procedure,as observed in both the training and validation cohorts.Additionally,in terms of predicting the long-term survival rate,the performance of the NDC model was significantly better than that of the other four models[Child-Pugh,model for end-stage liver disease(MELD),MELD-sodium and the Freiburg index of post-TIPS survival].CONCLUSION The NDC model can accurately predict long-term survival after the TIPS procedure in patients with hepatitis cirrhosis,help identify high-risk patients and guide follow-up management after TIPS implantation.
文摘Airplanes are a social necessity for movement of humans,goods,and other.They are generally safe modes of transportation;however,incidents and accidents occasionally occur.To prevent aviation accidents,it is necessary to develop a machine-learning model to detect and predict commercial flights using automatic dependent surveillance–broadcast data.This study combined data-quality detection,anomaly detection,and abnormality-classification-model development.The research methodology involved the following stages:problem statement,data selection and labeling,prediction-model development,deployment,and testing.The data labeling process was based on the rules framed by the international civil aviation organization for commercial,jet-engine flights and validated by expert commercial pilots.The results showed that the best prediction model,the quadratic-discriminant-analysis,was 93%accurate,indicating a“good fit”.Moreover,the model’s area-under-the-curve results for abnormal and normal detection were 0.97 and 0.96,respectively,thus confirming its“good fit”.

文摘Objective:To construct a risk prediction model for fall in patients with maintenance hemodialysis(MHD)and to verify the prediction effect of the model.Methods:From June 2020 to December 2020,307 patients who underwent MHD in a tertiary hospital in Chengdu were divided into a fall group(32 cases)and a non-fall group(275 cases).Logistic regression analysis model was used to establish the influencing factors of the subjects.Hosmer–Lemeshow and receiver operating characteristic(ROC)curve were used to test the goodness of fit and predictive effect of the model,and 104 patients were again included in the application research of the model.Results:The risk factors for fall were history of falls in the past year(OR=3.951),dialysis-related hypotension(OR=6.949),time up and go(TUG)test(OR=4.630),serum albumin(OR=0.661),frailty(OR=7.770),and fasting blood glucose(OR=1.141).Hosmer–Lemeshow test was P=0.475;the area under the ROC curve was 0.907;the Youden index was 0.642;the sensitivity was 0.843;and the specificity was 0.799.Conclusions:The risk prediction model constructed in this study has a good effect and can provide references for clinical screening of fall risks in patients with MHD.

文摘BACKGROUND Colorectal cancer(CRC)is one of the most prevalent and lethal malignant tumors worldwide.Currently,surgical intervention was the primary treatment modality for CRC.However,increasing studies have revealed that CRC patients may experience postoperative cognitive dysfunction(POCD).AIM To establish a risk prediction model for POCD in CRC patients and investigate the preventive value of dexmedetomidine(DEX).METHODS A retrospective analysis was conducted on clinical data from 140 CRC patients who underwent surgery at the People’s Hospital of Qian Nan from February 2020 to May 2024.Patients were allocated into a modeling group(n=98)and a validation group(n=42)in a 7:3 ratio.General clinical data were collected.Additionally,in the modeling group,patients who received DEX preoperatively were incorporated into the observation group(n=54),while those who did not were placed in the control group(n=44).The incidence of POCD was recorded for both cohorts.Data analysis was performed using statistical product and service solutions 20.0,with t-tests orχ2 tests employed for group comparisons based on the data type.Least absolute shrinkage and selection operator regression was applied to identify influencing factors and reduce the impact of multicollinear predictors among variables.Multivariate analysis was carried out using Logistic regression.Based on the identified risk factors,a risk prediction model for POCD in CRC patients was developed,and the predictive value of these risk factors was evaluated.RESULTS Significant differences were observed between the cognitive dysfunction group and the non-cognitive dysfunction group in diabetes status,alcohol consumption,years of education,anesthesia duration,intraoperative blood loss,intraoperative hypoxemia,use of DEX during surgery,intraoperative use of vasoactive drugs,surgical time,systemic inflammatory response syndrome(SIRS)score(P<0.05).Multivariate Logistic regression analysis identified that diabetes[odds ratio(OR)=4.679,95%confidence interval(CI)=1.382-15.833],alcohol consumption(OR=5.058,95%CI:1.255-20.380),intraoperative hypoxemia(OR=4.697,95%CI:1.380-15.991),no use of DEX during surgery(OR=3.931,95%CI:1.383-11.175),surgery duration≥90 minutes(OR=4.894,95%CI:1.377-17.394),and a SIRS score≥3(OR=4.133,95%CI:1.323-12.907)were independent risk factors for POCD in CRC patients(P<0.05).A risk prediction model for POCD was constructed using diabetes,alcohol consumption,intraoperative hypoxemia,non-use of DEX during surgery,surgery duration,and SIRS score as factors.A receiver operator characteristic curve analysis of these factors revealed the model’s predictive sensitivity(88.56%),specificity(70.64%),and area under the curve(AUC)(AUC=0.852,95%CI:0.773-0.919).The model was validated using 42 CRC patients who met the inclusion criteria,demonstrating sensitivity(80.77%),specificity(81.25%),and accuracy(80.95%),and AUC(0.805)in diagnosing cognitive impairment,with a 95%CI:0.635-0.896.CONCLUSION Logistic regression analysis identified that diabetes,alcohol consumption,intraoperative hypoxemia,non-use of DEX during surgery,surgery duration,and SIRS score vigorously influenced the occurrence of POCD.The risk prediction model based on these factors demonstrated good predictive performance for POCD in CRC individuals.This study offers valuable insights for clinical practice and contributes to the prevention and management of POCD under CRC circumstances.
文摘Malaria remains a major public health challenge necessitating accurate predictive models to inform effective intervention strategies in Sierra Leone. This study compares the performance of Holt-Winters’ Exponential Smoothing, Harmonic, and Artificial Neural Network (ANN) models using data from January 2018 to December 2023, incorporating both historical case records from Sierra Leone’s Health Management Information System (HMIS) and meteorological variables including humidity, precipitation, and temperature. The ANN model demonstrated superior performance, achieving a Mean Absolute Percentage Error (MAPE) of 4.74% before including climatic variables. This was further reduced to 3.9% with the inclusion of climatic variables, outperforming traditional models like Holt-Winters and Harmonic, which yielded MAPEs of 22.53% and 17.90% respectively. The ANN’s success is attributed to its ability to capture complex, non-linear relationships in the data, particularly when enhanced with relevant climatic variables. Using the optimized ANN model, we forecasted malaria cases for the next 24 months, predicting a steady increase from January 2024 to December 2025, with seasonal peaks. This study underscores the potential of machine learning approaches, particularly ANNs, in epidemiological modelling and highlights the importance of integrating environmental factors into malaria prediction models, recommending the ANN model for informing more targeted and efficient malaria control strategies to improve public health outcomes in Sierra Leone and similar settings.
文摘Time series forecasting is essential for generating predictive insights across various domains, including healthcare, finance, and energy. This study focuses on forecasting patient health data by comparing the performance of traditional linear time series models, namely Autoregressive Integrated Moving Average (ARIMA), Seasonal ARIMA, and Moving Average (MA) against neural network architectures. The primary goal is to evaluate the effectiveness of these models in predicting healthcare outcomes using patient records, specifically the Cancerpatient.xlsx dataset, which tracks variables such as patient age, symptoms, genetic risk factors, and environmental exposures over time. The proposed strategy involves training each model on historical patient data to predict age progression and other related health indicators, with performance evaluated using Mean Squared Error (MSE) and Root Mean Squared Error (RMSE) metrics. Our findings reveal that neural networks consistently outperform ARIMA and SARIMA by capturing non-linear patterns and complex temporal dependencies within the dataset, resulting in lower forecasting errors. This research highlights the potential of neural networks to enhance predictive accuracy in healthcare applications, supporting better resource allocation, patient monitoring, and long-term health outcome predictions.

文摘BACKGROUND Breast cancer(BC)is the second most common malignancy globally.Young and middle-aged patients face more pressures from diagnosis,treatment,costs,and psychological issues like self-image concerns,social barriers,and professional challenges.Compared to other age groups,they have higher recurrence rates,lower survival rates,and increased risk of depression.Research is lacking on factors influencing depressive symptoms and predictive models for this age group.AIM To analyze factors influencing depressive symptoms in young/middle-aged BC patients and construct a depression risk predictive model.METHODS A total of 360 patients undergoing BC treatment at two tertiary hospitals in Jiangsu Province,China from November 2023 to April 2024 were included in the study.Participants were surveyed using a general information questionnaire,the patient health questionnaire depression scale,the visual analog scale for pain,the revised family support scale,and the long form of the international physical activity questionnaire.Univariate and multivariate analyses were conducted to identify the factors affecting depression in middle-aged and young BC patients,and a predictive model for depression risk was developed based on these findings.RESULTS Among the 360 middle-aged and young BC patients,the incidence rate of depressive symptoms was 38.61%(139/360).Multivariate analysis revealed that tumor grade,patient’s monthly income,pain score,family support score,and physical activity score were factors influencing depression in this patient group(P<0.05).The risk prediction model constructed based on these factors yielded an area under the receiver operating characteristic curve of 0.852,with a maximum Youden index of 0.973,sensitivity of 86.80%,specificity of 89.50%,and a diagnostic odds ratio of 0.552.The Hosmer-Lemeshow test for goodness of fit indicated an adequate model fit(χ^(2)=0.360,P=0.981).CONCLUSION The constructed predictive model demonstrates good predictive performance and can serve as a reference for medical professionals to early identify high-risk patients and implement corresponding preventive measures to decrease the incidence of depressive symptoms in this population.

文摘In this article,we comment on the article published by Yu et al.By employing LASSO regression and Cox proportional hazard models,the article identified nine significant variables affecting survival,including body mass index,Karnofsky performance status,and tumor-node-metastasis staging.We firmly concur with Yu et al regarding the vital significance of clinical prediction models(CPMs),including logistic regression and Cox regression for assessment in esophageal carcinoma(EC).However,the nomogram's limitations and the complexities of integrating genetic factors pose challenges.The integration of immunological data with advanced statistics offers new research directions.High-throughput sequencing and big data,facilitated by machine learning,have revolutionized cancer research but require substantial computational resources.The future of CPMs in EC depends on leveraging these technologies to improve predictive accuracy and clinical application,addressing the need for larger datasets,patientreported outcomes,and regular updates for clinical relevance.

文摘BACKGROUND The risk factors and prediction models for diabetic foot(DF)remain incompletely understood,with several potential factors still requiring in-depth investigations.AIM To identify risk factors for new-onset DF and develop a robust prediction model for hospitalized patients with type 2 diabetes.METHODS We included 6301 hospitalized patients with type 2 diabetes from January 2016 to December 2021.A univariate Cox model and least absolute shrinkage and selection operator analyses were applied to select the appropriate predictors.Nonlinear associations between continuous variables and the risk of DF were explored using restricted cubic spline functions.The Cox model was further employed to evaluate the impact of risk factors on DF.The area under the curve(AUC)was measured to evaluate the accuracy of the prediction model.RESULTS Seventy-five diabetic inpatients experienced DF.The incidence density of DF was 4.5/1000 person-years.A long duration of diabetes,lower extremity arterial disease,lower serum albumin,fasting plasma glucose(FPG),and diabetic nephropathy were independently associated with DF.Among these risk factors,the serum albumin concentration was inversely associated with DF,with a hazard ratio(HR)and 95%confidence interval(CI)of 0.91(0.88-0.95)(P<0.001).Additionally,a U-shaped nonlinear relationship was observed between the FPG level and DF.After adjusting for other variables,the HRs and 95%CI for FPG<4.4 mmol/L and≥7.0 mmol/L were 3.99(1.55-10.25)(P=0.004)and 3.12(1.66-5.87)(P<0.001),respectively,which was greater than the mid-range level(4.4-6.9 mmol/L).The AUC for predicting DF over 3 years was 0.797.CONCLUSION FPG demonstrated a U-shaped relationship with DF.Serum albumin levels were negatively associated with DF.The prediction nomogram model of DF showed good discrimination ability using diabetes duration,lower extremity arterial disease,serum albumin,FPG,and diabetic nephropathy(Clinicaltrial.gov NCT05519163).

文摘This study focuses on empirical modeling of the strength characteristics of urban soils contaminated with heavy metals using machine learning tools and their subsequent stabilization with ordinary Portland cement(OPC).For dataset collection,an extensive experimental program was designed to estimate the unconfined compressive strength(Qu)of heavy metal-contaminated soils collected from awide range of land use pattern,i.e.residential,industrial and roadside soils.Accordingly,a robust comparison of predictive performances of four data-driven models including extreme learning machines(ELMs),gene expression programming(GEP),random forests(RFs),and multiple linear regression(MLR)has been presented.For completeness,a comprehensive experimental database has been established and partitioned into 80%for training and 20%for testing the developed models.Inputs included varying levels of heavy metals like Cd,Cu,Cr,Pb and Zn,along with OPC.The results revealed that the GEP model outperformed its counterparts:explaining approximately 96%of the variability in both training(R2=0.964)and testing phases(R^(2)=0.961),and thus achieving the lowest RMSE and MAE values.ELM performed commendably but was slightly less accurate than GEP whereas MLR had the lowest performance metrics.GEP also provided the benefit of traceable mathematical equation,enhancing its applicability not just as a predictive but also as an explanatory tool.Despite its insights,the study is limited by its focus on a specific set of heavy metals and urban soil samples of a particular region,which may affect the generalizability of the findings to different contamination profiles or environmental conditions.The study recommends GEP for predicting Qu in heavy metal-contaminated soils,and suggests further research to adapt these models to different environmental conditions.
文摘Accurate prediction of nurse demand plays a crucial role in efficiently planning the healthcare workforce,ensuring appropriate staffing levels,and providing high-quality care to patients.The intricacy and variety of contemporary healthcare systems and a growing patient populace call for advanced forecasting models.Factors like technological advancements,novel treatment protocols,and the increasing prevalence of chronic illnesses have diminished the efficacy of traditional estimation approaches.Novel forecasting methodologies,including time-series analysis,machine learning,and simulation-based techniques,have been developed to tackle these challenges.Time-series analysis recognizes patterns from past data,whereas machine learning uses extensive datasets to uncover concealed trends.Simulation models are employed to assess diverse scenarios,assisting in proactive adjustments to staffing.These techniques offer distinct advantages,such as the identification of seasonal patterns,the management of large datasets,and the ability to test various assumptions.By integrating these sophisticated models into workforce planning,organizations can optimize staffing,reduce financial waste,and elevate the standard of patient care.As the healthcare field progresses,the utilization of these predictive models will be pivotal for fostering adaptable and resilient workforce management.
文摘BACKGROUND Colorectal polyps are precancerous diseases of colorectal cancer.Early detection and resection of colorectal polyps can effectively reduce the mortality of colorectal cancer.Endoscopic mucosal resection(EMR)is a common polypectomy proce-dure in clinical practice,but it has a high postoperative recurrence rate.Currently,there is no predictive model for the recurrence of colorectal polyps after EMR.AIM To construct and validate a machine learning(ML)model for predicting the risk of colorectal polyp recurrence one year after EMR.METHODS This study retrospectively collected data from 1694 patients at three medical centers in Xuzhou.Additionally,a total of 166 patients were collected to form a prospective validation set.Feature variable screening was conducted using uni-variate and multivariate logistic regression analyses,and five ML algorithms were used to construct the predictive models.The optimal models were evaluated based on different performance metrics.Decision curve analysis(DCA)and SHapley Additive exPlanation(SHAP)analysis were performed to assess clinical applicability and predictor importance.RESULTS Multivariate logistic regression analysis identified 8 independent risk factors for colorectal polyp recurrence one year after EMR(P<0.05).Among the models,eXtreme Gradient Boosting(XGBoost)demonstrated the highest area under the curve(AUC)in the training set,internal validation set,and prospective validation set,with AUCs of 0.909(95%CI:0.89-0.92),0.921(95%CI:0.90-0.94),and 0.963(95%CI:0.94-0.99),respectively.DCA indicated favorable clinical utility for the XGBoost model.SHAP analysis identified smoking history,family history,and age as the top three most important predictors in the model.CONCLUSION The XGBoost model has the best predictive performance and can assist clinicians in providing individualized colonoscopy follow-up recommendations.

文摘BACKGROUND The discrepancy between endoscopic biopsy pathology and the overall pathology of gastric low-grade intraepithelial neoplasia(LGIN)presents challenges in developing diagnostic and treatment protocols.AIM To develop a risk prediction model for the pathological upgrading of gastric LGIN to aid clinical diagnosis and treatment.METHODS We retrospectively analyzed data from patients newly diagnosed with gastric LGIN who underwent complete endoscopic resection within 6 months at the First Medical Center of Chinese People’s Liberation Army General Hospital between January 2008 and December 2023.A risk prediction model for the pathological progression of gastric LGIN was constructed and evaluated for accuracy and clinical applicability.RESULTS A total of 171 patients were included in this study:93 patients with high-grade intraepithelial neoplasia or early gastric cancer and 78 with LGIN.The logistic stepwise regression model demonstrated a sensitivity and specificity of 0.868 and 0.800,respectively,while the least absolute shrinkage and selection operator(LASSO)regression model showed sensitivity and specificity values of 0.842 and 0.840,respectively.The area under the curve(AUC)for the logistic model was 0.896,slightly lower than the AUC of 0.904 for the LASSO model.Internal validation with 30%of the data yielded AUC scores of 0.908 for the logistic model and 0.905 for the LASSO model.The LASSO model provided greater utility in clinical decision-making.CONCLUSION A risk prediction model for the pathological upgrading of gastric LGIN based on white-light and magnifying endoscopic features can accurately and effectively guide clinical diagnosis and treatment.

文摘BACKGROUND Patients with chronic obstructive pulmonary disease(COPD)frequently experience exacerbations requiring multiple hospitalizations over prolonged disease courses,which predispose them to generalized anxiety disorder(GAD).This comorbidity exacerbates breathing difficulties,activity limitations,and social isolation.While previous studies predominantly employed the GAD 7-item scale for screening,this approach is somewhat subjective.The current literature on predictive models for GAD risk in patients with COPD is limited.AIM To construct and validate a GAD risk prediction model to aid healthcare professionals in preventing the onset of GAD.METHODS This retrospective analysis encompassed patients with COPD treated at our institution from July 2021 to February 2024.The patients were categorized into a modeling(MO)group and a validation(VA)group in a 7:3 ratio on the basis of the occurrence of GAD.Univariate and multivariate logistic regression analyses were utilized to construct the risk prediction model,which was visualized using forest plots.The model’s performance was evaluated using Hosmer-Lemeshow(H-L)goodness-of-fit test and receiver operating characteristic(ROC)curve analysis.RESULTS A total of 271 subjects were included,with 190 in the MO group and 81 in the VA group.GAD was identified in 67 patients with COPD,resulting in a prevalence rate of 24.72%(67/271),with 49 cases(18.08%)in the MO group and 18 cases(22.22%)in the VA group.Significant differences were observed between patients with and without GAD in terms of educational level,average household income,smoking history,smoking index,number of exacerbations in the past year,cardiovascular comorbidities,disease knowledge,and personality traits(P<0.05).Multivariate logistic regression analysis revealed that lower education levels,household income<3000 China yuan,smoking history,smoking index≥400 cigarettes/year,≥two exacerbations in the past year,cardiovascular comorbidities,complete lack of disease information,and introverted personality were significant risk factors for GAD in the MO group(P<0.05).ROC analysis indicated that the area under the curve for predicting GAD in the MO and VA groups was 0.978 and 0.960.The H-L test yieldedχ^(2) values of 6.511 and 5.179,with P=0.275 and 0.274.Calibration curves demonstrated good agreement between predicted and actual GAD occurrence risks.CONCLUSION The developed predictive model includes eight independent risk factors:Educational level,household income,smoking history,smoking index,number of exacerbations in the past year,presence of cardiovascular comorbidities,level of disease knowledge,and personality traits.This model effectively predicts the onset of GAD in patients with COPD,enabling early identification of high-risk individuals and providing a basis for early preventive interventions by nursing staff.
文摘Background: Pancreatic cancer is one of the most lethal malignancies, with postoperative recurrence severely affecting patient survival and prognosis. This study aims to develop and validate a clinical prediction model for postoperative recurrence in pancreatic cancer patients, incorporating multiple preoperative, intraoperative, and postoperative factors to assist clinical decision-making. Methods: A retrospective study was conducted on 216 patients who underwent surgical treatment for pancreatic malignancy at the First Affiliated Hospital of Chongqing Medical University between January 2015 and January 2023. An independent external validation cohort of 76 patients from the Second Affiliated Hospital of Chongqing Medical University was used to validate the model. Seven independent risk factors for postoperative recurrence were identified through univariate and multivariate Cox regression analyses. The model’s performance was evaluated using the concordance index (C-index) and ROC curves, and its accuracy and clinical value were assessed using calibration curves and decision curve analysis (DCA). Results: The predictive model demonstrated good discriminatory power, with a C-index of 0.72 in the training cohort and 0.66 in the validation cohort. The ROC curves for predicting recurrence at 3, 6, and 12 months postoperatively showed AUC values ranging from 0.72 to 0.83, indicating strong predictive value. Calibration curves and DCA confirmed the model’s accuracy and clinical utility. Conclusion: This study successfully developed and validated a clinical prediction model that incorporates seven independent risk factors for postoperative recurrence in pancreatic cancer. The model provides a useful tool for predicting recurrence risk, aiding in the identification of high-risk patients, and informing clinical decision-making.

文摘Objective:To evaluate the use of infrared thermography technology for objective and quantitative syndrome differentiation and treatment in traditional Chinese medicine(TCM),specifically in patients with Chaihu Guizhi Ganjiang Decoction syndrome.Methods:Data were collected from over 100 patients diagnosed with Chaihu Guizhi Ganjiang Decoction syndrome at Professor Li Leyu’s endocrinology clinic,Zhongshan Hospital of Traditional Chinese Medicine,Guangdong Province,between April 2021 and April 2022.Body surface temperature data were obtained using the MTI-EXPRO-2013-B infrared thermography system.Principal component analysis(PCA)was applied to differentiate temperature distribution characteristics between genders,and a neural network prediction model was constructed for syndrome diagnosis.Results:Infrared thermography effectively captured surface temperature characteristics of patients with Chaihu Guizhi Ganjiang Decoction syndrome.PCA identified one principal component with a variance explanation rate of 73.953%for females and two principal components with a cumulative variance explanation rate of 77.627%for males.The neural network model demonstrated high predictive performance,with an area under the ROC curve of 0.9743 for the training set and 0.9889 for the validation set.Sensitivity was 1,specificity 0.8636,precision 0.8846,accuracy 0.9333,and the F1 score 0.9388.Conclusion:Infrared thermography provides an innovative,objective,and quantitative method for syndrome differentiation and treatment in TCM.It represents a significant advancement in transitioning from traditional empirical approaches to modern,visualized,and precise diagnosis and treatment.This study underscores the potential of integrating advanced technologies in TCM for enhanced clinical application and modernization.

文摘BACKGROUND:Intracranial hemorrhage (ICH),a severe complication among adults receiving extracorporeal membrane oxygenation (ECMO),is often related to poor outcomes.This study aimed to establish a predictive model for ICH in adults receiving ECMO treatment.METHODS:Adults who received ECMO between January 2017 and June 2022 were the subjects of a single-center retrospective study.Patients under the age of 18 years old,with acute ICH before ECMO,with less than 24 h of ECMO support,and with incomplete data were excluded.ICH was diagnosed by a head computed tomography scan.The outcomes included the incidence of ICH,in-hosptial mortality and 28-day mortality.Multivariate logistic regression analysis was used to identify relevant risk factors of ICH,and a predictive model of ICH with a nomogram was constructed.RESULTS:Among the 227 patients included,22 developed ICH during ECMO.Patients with ICH had higher in-hospital mortality (90.9%vs.47.8%,P=0.001) and higher 28-day mortality (81.8%vs.47.3%,P=0.001) than patients with non-ICH.ICH was associated with decreased grey-white-matter ratio (GWR)(OR=0.894,95%CI:0.841–0.951,P<0.001),stroke history (OR=4.265,95%CI:1.052–17.291,P=0.042),fresh frozen plasma (FFP) transfusion (OR=1.208,95%CI:1.037–1.408,P=0.015)and minimum platelet (PLT) count during ECMO support (OR=0.977,95%CI:0.958–0.996,P=0.019).The area under the receiver operating characteristic curve of the ICH predictive model was 0.843 (95%CI:0.762–0.924,P<0.001).CONCLUSION:ECMO-treated patients with ICH had a higher risk of death.GWR,stroke history,FFP transfusion,and the minimum PLT count were independently associated with ICH,and the ICH predictive model showed that these parameters performed well as diagnostic tools.
