Real-world evidence on the efficacy and safety of vonoprazanamoxicillin dual therapy for Helicobacter pylori treatment in elderly patients

BACKGROUND A dual therapy regimen containing amoxicillin is a common treatment option for the eradication of Helicobacter pylori(H.pylori).While substantial research supports the efficacy and safety of vonoprazan and amoxicillin(VA)dual therapy in the general population,there is still a lack of studies specifically focusing on its safety in elderly patients.AIM To evaluate efficacy and safety of VA dual therapy as first-line or rescue treatment for H.pylori in elderly patients.METHODS As a real-world retrospective study,data were collected from elderly patients aged 60 years and above who accepted VA dual therapy(vonoprazan 20 mg twice daily+amoxicillin 1000 mg thrice daily for 14 days)for H.pylori eradication in the Department of Gastroenterology at Peking University First Hospital between June 2020 and January 2024.H.pylori status was evaluated by^(13)C-urease breath test 6 weeks after treatment.All adverse events(AEs)during treatment were recorded.RESULTS In total,401 cases were screened.Twenty-one cases were excluded due to loss to follow-up,lack of re-examination,or unwillingness to take medication.The total of 380 included cases comprised 250 who received VA dual therapy as first-line treatment and 130 who received VA dual therapy as rescue treatment.H.pylori was successfully eradicated in 239 cases(95.6%)in the first-line treatment group and 116 cases(89.2%)in the rescue treatment group.The overall incidence of AEs was 9.5%for both groups.Specifically,9.2%of patients experienced an AE in the first-line treatment group and 10.0%in the rescue treatment group.Five patients discontinued treatment due to AE,with a discontinuation rate of 1.3%.No serious AE occurred.CONCLUSION The VA dual therapy regimen as a first-line treatment and a rescue therapy was effective and safe for elderly patients aged 60 and older. 展开更多

BACKGROUND Administering anesthesia to elderly patients undergoing gastroenteroscopy necessitates careful attention due to age-related physiological changes and an increased risk of complications.AIM To analyze the research trends in anesthesia management for elderly patients undergoing gastroenteroscopy.METHODS We performed a literature search using the Web of Science database to identify articles published between 2004 and 2023.Bibliometric and visual analyses were conducted using CiteSpace,R,and VOSviewer to explore the current research landscape of anesthesia administration in painless gastroenteroscopy for elderly patients and to identify future research directions by examining trends and emerging hotspots in this domain.RESULTS A total of 800 articles were examined,revealing a rising trend in annual pub-lication counts.The United States led with 181 articles,followed by China with 112,collectively contributing over 35%of the studies among the top ten countries.The majority of publications appeared in the United States journals,with the top three being Gastrointestinal Endoscopy[impact factor(IF)=7.7,H-index=26],Digestive Diseases and Sciences(IF=3.1),and Endoscopy(IF=9.3).Six primary research clusters were identified:Obstructive sleep apnea and airway manage-ment,surveillance and risk factors,colorectal cancer examination and treatment,sedation and safety of propofol and midazolam,patient satisfaction,and mortality and complications.These findings underscore the pivotal focus areas in anesthesia for elderly patients undergoing gastroenteroscopy.CONCLUSION A comprehensive understanding of current research trends and hotspots will aid anesthesiologists in developing more evidence-based practices,thereby improving the safety and outcomes for elderly patients undergoing gastroenteroscopy. 展开更多

Background: ACS is a leading cause of mortality worldwide, but it is unknown whether the advanced age of patients constitutes a prognostic factor. Objective: The present study compared the burden of risk factors and the prognosis of ACS in older and younger patients in order to improve their management. Methods. A retrospective analysis of a series of ACS patients admitted from January to December 2022 to intensive care unit of the cardiology department at the South Francilian Hospital Center. Results: The files of 551 patients including 60 elderly subjects (10.9%) were analyzed. Average age was 81.7 ± 6.1 years for the elderly, 54.5 ± 11.9 years for the younger patients. Men predominated in both age categories (p = 0.014) and were older than women (59.2 ± 14.3 vs 53.1 ± 13.2 years;p = 0.014). Obesity (p = 0.007), smoking (p = 0.002) and dyslipidemia (p = 0.022) were the cardiovascular risk factors found more in elderly patients, hypertension (p < 0.001) and diabetes (p < 0.001) in the youngest. NSTEMI (66.3%) predominated in both the elderly (55.0%) and the younger (60.8%) subjects. Treatment consisted of immediate angioplasty in 66.1%, including 61.7% elderly subjects and 66.6% younger subjects (p = 0.224). Death was significantly associated with the severity of coronary lesions (p < 0.001) and chronic renal failure (p = 0.006), with no significant influence of age. Conclusion: The proportion of elderly subjects with ACS is not negligible. Comorbidities increase the prognosis, without age per se being associated with death. 展开更多

BACKGROUND With an increase in the elderly population,the frequency of hospitalizations in recent years has also risen at a rapid pace.This,in turn,has resulted in poor outcomes and costly treatments.Hospitalization rates increase in elderly patients due to a decline in glomerular filtration rate(GFR).AIM To investigate the connection between GFR and comorbidity and reasons for hospitalization in elderly patients.METHODS We analyzed patients aged 75 years and over who were admitted to the internal medicine clinic of a tertiary hospital in Eskisehir.At admission,we calculated GFR values using the Modification of Diet in Renal Disease study formula and classified them into six categories:G1,G2,G3a,G3b,G4,and G5.We analyzed associations with hospitalization diagnoses and comorbidity factors.RESULTS The average age of the patients was 80.8 years(±4.5 years).GFR was 57.287±29.5 mL/kg/1.73 m2 in women and 61.3±31.5 mL/kg/1.73 m2 in men(P=0.106).Most patients were admitted to the hospital at G2 stage(32.8%).The main reasons for hospitalization were anemia(34.4%and 28.6%)and malnutrition(20.9%and 20.8%)in women and men,respectively(P=0.078).The most frequent comor-bidity leading to hospitalization was arterial hypertension(n=168,28%),fo-llowed by diabetes(n=166,27.7%)(P=0.001).CONCLUSION When evaluating geriatric patients,low GFR alone does not provide sufficient information.Patients’comorbid factors should also be taken into account.There is no association between low GFR during hospitalization and hospitalization-Hamarat H.Aging and GFR related diagnoses.Knowing the GFR value before hospitalization will be more informative in such studies. 展开更多

Biomechanical study of the visual system by ocular response analyzer investigates the inter-structural biological relationships,mechanics,and function of the visual system.This review aimed to investigate the changes in corneal biomechanical parameters with age and sex.The articles published in PubMed between 2000 and 2021 were investigated and critiqued,and valid scientific evidence was collected,reviewed and concluded according to the inclusion and exclusion criteria.Most studies showed that corneal biomechanical changes occur infrequently in children up to the age of 20y,and with increasing age and wider age range,there was a significant decrease in corneal biomechanical indices,especially corneal hysteresis.In children and adults,most studies have shown that these biomechanical indicators,especially corneal resistance factor,were higher in females.Although hormonal changes may contribute to this finding,the role of axial length and other biometric indicators should not be ignored.The axial length,the intraocular pressure,and the corneal thickness are other factors associated with biomechanical parameters that should be taken into account in clinical diagnosis and management especially for patients undergoing refractive surgery as well as keratoconus patients. 展开更多

Bivalirudin for anticoagulation in elderly acute coronary syndrome:Effects on myocardial microcirculation and adverse events

The management of acute coronary syndrome(ACS)in older patients remains challenging because standard anticoagulants often fail to yield optimal outcomes.Bivalirudin,a direct inhibitor of thrombin,serves as an alternative to traditional therapies.This drug is particularly effective in enhancing myocardial microcircu-lation and reducing adverse events after clinical interventions.The present article explores the findings of a recent study that highlighted the clinical benefits of bivalirudin by investigating its effects on myocardial microcirculation and adverse cardiac events after percutaneous coronary intervention in older patients with ACS.Compared with unfractionated heparin,bivalirudin markedly reduced the emergency response time and improved cardiac function indicators.It further mitigated the risks of cardiovascular events and recurrent myocardial infarctions.These findings suggest that bivalirudin can enhance myocardial perfusion and reduce bleeding complications,thus serving as a safe,effective anticoagulation agent for older patients with ACS.Nonetheless,further large-scale,high-quality trials are needed to establish optimal usage guidelines and assess long-term outcomes.Integrating bivalirudin into ACS treatment protocols for older patients may help optimize patient care,balancing efficacy and safety.Continual research and consensus building are necessary for the widespread clinical application of bivalirudin and the improvement of ACS outcomes in older patients. 展开更多

Objective:To evaluate the effectiveness of continuous nursing interventions in elderly patients with COPD in the stable phase and frailty.Methods:Sixty elderly patients with COPD in the stable phase and frailty,treated between January 2024 and August 2024,were selected as the study subjects.Patients were randomly divided into two groups(30 each)using a drawing method.Patients who drew a black token were assigned to the intervention group and received continuous nursing interventions,while those who drew a red token were assigned to the nursing group and received standard nursing care.The quality of care between the groups was compared.Results:Pulmonary function indicators in the intervention group were significantly better than those in the nursing group(P<0.05).Immune function in the intervention group was also significantly higher than in the nursing group(P<0.05).Before the intervention,there was no significant difference in self-care ability between the two groups(P>0.05).However,post-intervention,both groups showed improved self-care abilities,with the intervention group scoring significantly higher than the nursing group(P<0.05).Similarly,before the intervention,there was no significant difference in quality-of-life scores(P>0.05),but post-intervention,both groups exhibited increased scores,with the intervention group outperforming the nursing group significantly(P<0.05).Conclusion:Continuous nursing interventions significantly improve outcomes in elderly patients with COPD in the stable phase and frailty,warranting broader implementation. 展开更多

BACKGROUND A significant association between increased age and an increased risk of metachronous gastric cancer(MGC)following curative endoscopic submucosal dissection(ESD)has previously been reported.AIM To determine risk factors for the metachronous occurrence of early gastric cancer(EGC)in elderly individuals.METHODS This retrospective cohort study comprised 653 elderly patients(aged≥65 years)who underwent curative ESD for EGC between January 2014 and June 2020 at Nanjing Drum Tower Hospital.Comprehensive analyses were conducted to compare lifestyle habits,comorbidities,and Helicobacter pylori(H.pylori)infections as potential indicators.RESULTS During a median follow-up of 38 months,46 patients(7.0%,20.46/1000 person-years)developed MGC in the elderly cohort.The cumulative incidences of MGC at 2,3,and 5 years were 3.3%,5.3%,and 11.5%,respectively.In multivariate Cox regression analyses,the independent risk factors for MGC included metabolic dysfunctionassociated steatotic liver disease(MASLD)[hazard ratio(HR)=2.44,95%confidence interval(CI):1.15-5.17],persistent H.pylori infection(HR=10.38,95%CI:3.36-32.07),severe mucosal atrophy(HR=2.71,95%CI:1.45-5.08),and pathological differentiation of EGC(well/moderately differentiated vs poorly differentiated:HR=10.18,95%CI:1.30-79.65).Based on these risk factors,a risk stratification system was developed to categorize individuals into low(0-1 point),intermediate(2-3 points),and high(4-8 points)risk categories for MGC,with cumulative incidence rates of 12.3%,21.6%,and 45%,respectively.CONCLUSION Among elderly individuals,MASLD,persistent H.pylori infection,severe mucosal atrophy,and well/moderately differentiated EGC were associated with an increased risk of MGC.Elderly patients are recommended to adopt healthy lifestyle practices,and undergo regular endoscopic screening and H.pylori testing after curative ESD for EGC. 展开更多

Objective:To investigate the incidence of delirium in elderly patients with Stanford-type B aortic dissection and analyze its risk factors.Methods:A convenience sample of 767 elderly patients with Stanford-type B aortic dissection admitted to the ICU from January 2020 to December 2023 was selected.Data were collected using a delirium-related questionnaire and the Confusion Assessment Method for the Intensive Care Unit(CAM-ICU).Results:The incidence of delirium in elderly Stanford B aortic dissection patients was 23.73%.Logistic regression analysis showed that gender,length of stay in the ICU,and duration of sedative drug use were independent risk factors for delirium in elderly patients(P<0.05).The model likelihood ratio test x^(2)=28.462,P<0.001;Hosmer-Lemeshow goodness-of-fit test x^(2)=0.715,P=0.878.Conclusion:The incidence of delirium in elderly patients with Stanford-type B aortic dissection is relatively low.Medical staff should conduct adequate and effective preoperative assessment according to the condition of elderly Stanford-type B aortic dissection patients,and use analgesic and sedative drugs reasonably to create a good treatment environment for patients,thereby minimizing the incidence of delirium in elderly patients with Stanford-type B aortic dissection as much as possible. 展开更多

Conjunctival microbiota variations in a subset of middleaged and elderly individuals from Beijing,China

AIM:To isolate and identify the conjunctival microbiota of cataract patients and analyze the associated influencing factors.METHODS:This study recruited 216 participants(216 eyes)from April 2022 to July 2022.Under the condition of no antibiotic use prior to cataract surgery,sterile swabs were used to collect samples from the lower conjunctival sac.Bacterial cultures were then conducted,followed by species identification through 16S rDNA gene sequencing.Clinical factors associated with positive or negative bacterial isolation rates were analyzed,including age,gender,meibomian gland dysfunction(MGD),history of hypertension,history of diabetes,history of cancer,history of infectious diseases and the habit of wearing masks.RESULTS:Among the 216 eyes,78 eyes yielded isolates,with an isolation rate of 36.11%,detecting a total of 122 strains.Gram-positive rods accounted for 49.18%(60 strains),gram-positive cocci accounted for 45.08%(55 strains),gram-negative bacteria accounted for 4.92%(6 strains),and fungi accounted for 0.82%(1 strain).This study found that the most abundant genera in the conjunctival sac were Corynebacterium(42.62%),Staphylococcus(31.15%),Micrococcus(9.84%),Acinetobacter(4.10%),and Bacillus(3.28%).Furthermore,age(P=0.006),gender(P=0.039),diabetes(P=0.003),history of infectious diseases(P=0.02),and duration of mask replacement(P<0.001)were important factors influencing the positive bacterial culture of the conjunctival microbiota.Although hypertensive patients exhibited a higher isolation rate of conjunctival bacteria,it did not reach statistical significance,and the history of cancer did not affect the isolation rate of the conjunctival microbial community in cataract patients before surgery.CONCLUSION:Potential changes are observed in the conjunctival microbiota among a sample of middleaged and elderly individuals from Beijing,China.Notably,an increased isolation rate of Corynebacterium and Micrococcus is detected,suggesting a possible change in the microbial balance that requires further investigation and attention from the ophthalmological community.Advanced age,female gender,MGD,diabetes,a recent history of infectious diseases,and inadequate mask-wearing habits are potentially significant factors associated with the conjunctival microbiota.These factors should be considered in the development of strategies to prevent perioperative infections in cataract surgery patients. 展开更多

Isletβ-cell function preservation by different anti-diabetic treatments in Chinese elderly patients with type 2 diabetes mellitus

BACKGROUND The preservation of isletβ-cell function in elderly patients with type 2 diabetes mellitus(T2DM)is a top priority for diabetic control.AIM To assess the preservation of isletβ-cell function among elderly Chinese patients with T2DM after different anti-diabetic treatments.METHODS In this longitudinal observational study,elderly patients with T2DM treated with insulin,oral antidiabetic drugs or a combination of both were enrolled to disclose their isletβ-cell function between baseline and follow-up.Isletβ-cell function was determined by the plasma Homeostasis Model forβ-cell function(HOMA-β),Cpeptide and area under the curve(AUC)based on oral glucose tolerance test.Changes inβ-cell function(decrement or increment from baseline)between different therapy groups were the outcomes.RESULTS In total,745 elderly patients(≥60 years)with T2DM[insulin monotherapy,n=105;oral anti-diabetic drugs(OAD)monotherapy,n=321;insulin plus OAD,n=319]had their baseline and follow-upβ-cell function assessed during a median observation period of 4.5 years(range,3.0-7.2 years).Overall,isletβ-cell function(HOMA-β,fasting Cpeptide,fasting insulin,AUCc-pep,AUCins,AUCc-pep/AUCglu,AUCins/AUCglu)consistently deteriorated over time regardless of the three different antidiabetic treatments.No statistical differences in decrement were observed among the three groups regarding the isletβ-cell function indices.All three groups showed an increased ratio of delayed insulin secretion response after 4.5 years of observation.CONCLUSION In Chinese elderly patients with T2DM,isletβ-cell function progressively declines regardless of insulin supplement or insulin plus OAD treatments. 展开更多

Objective:To analyze the characteristics of ambulatory blood pressure in elderly patients with hypertension and find out the risk factors of abnormal circadian rhythm.Methods:According to the circadian rhythm of patients'blood pressure,they were divided into Group A,Group B and Group C,and all the data of hypertension patients in this study were collected,including age,gender,BMI,smoking,drinking,basic diseases(diabetes,cerebrovascular disease,hyperlipidemia,etc.),fasting blood glucose,ambulatory blood pressure(24-hour mean systolic pressure,24-hour mean diastolic pressure,daytime mean systolic pressure and daytime mean diastolic pressure).Results:There were significant differences in mean systolic blood pressure and mean diastolic blood pressure at night among Group A,Group B and Group C(P<0.05).Age,hyperlipidemia and fasting blood glucose were risk factors for circadian rhythm abnormality(P<0.05),and 24-hour urinary sodium was a protective factor for circadian rhythm abnormality(P<0.05).Conclusion:Age,hyperlipidemia and fasting blood glucose are risk factors for circadian rhythm abnormality(P<0.05),and 24-hour urinary sodium is a protective factor for circadian rhythm abnormality(P<0.05). 展开更多

Objective:To investigate the clinical efficacy of levofloxacin combined with ambroxol in the treatment of elderly patients with chronic obstructive pulmonary disease(COPD)and pulmonary infection.Methods:A total of 80 elderly COPD patients with pulmonary infection,treated between December 2022 and November 2023,were randomly divided into a control group and an observation group,with 40 cases in each group.The control group was treated with levofloxacin hydrochloride,while the observation group received ambroxol hydrochloride injection in addition to the treatment in the control group.Laboratory indices(white blood cell count,procalcitonin,C-reactive protein,and apolipoprotein E levels),imaging-based pulmonary lesion absorption time,hospital stay,and incidence of adverse reactions were compared between the two groups.Results:After treatment,the biochemical indices of the observation group were significantly lower than those of the control group,with highly significant differences(P<0.001).Compared to the control group,the imaging-based pulmonary lesion absorption time and hospital stay of the observation group were significantly shorter(P<0.001).Additionally,the incidence of adverse reactions in the observation group was significantly lower than in the control group(P<0.05).Conclusion:Levofloxacin combined with ambroxol demonstrates advantages in improving biochemical indices,shortening imaging-based pulmonary lesion absorption time and hospital stay,and reducing adverse reaction rates in elderly COPD patients with pulmonary infection.It holds significant clinical application value. 展开更多

Objective: The health-related quality of life (HRQoL) theory was used to assess the health quality of elderly residents in Southwest China. This was done by using the European Five-Dimensional Health Scale in a comprehensive manner and by analysing the factors influencing it. The findings will provide new perspectives and ideas for improving the health-related quality of life of the elderly population and enhancing the precise health management of elderly residents. Methods: The response data of 1892 elderly residents in southwestern China were included in the analysis based on the CLHLS data. The factors influencing the occurrence of problems, EQ-VAS scores and health utility values were analysed by logistic regression, multiple linear regression and Tobit regression, respectively. Results: The primary health concerns among the elderly population in the Southwest region were limited ability to perform daily activities and pain or discomfort. These individuals exhibited an EQ-VAS self-assessment score of 66.51 ± 14.87 and a health utility value of 0.87 (0.70, 1.00). Gender, age, regular medical check-ups, exercise habits and the prevalence of chronic diseases are the main influencing factors. Conclusions: The health quality of elderly people in Southwest China needs to be improved, and a comprehensive management strategy can be adopted in terms of lifestyle management, health needs management and disease management to improve the quality of their healthy lives and promote the development of healthy ageing. 展开更多

Objective: To examine the daily life care, spiritual comfort, and fall safety care needs of the elderly in urban and rural areas of Tangshan, along with their influencing factors. Methods: From August 2022 to April 2023, an investigation was conducted among urban and rural elderly individuals aged over 75 years in Tangshan City using the Activities of Daily Living Scale, the Revised Community Elderly Fall Risk Assessment Tool, and the Loneliness Scale. Results: The study included 750 urban and 740 rural elderly individuals aged over 75 years. Matrix analysis revealed a significant proportion of fall safety care needs across various daily life and spiritual care requirements. Multiple factor analysis indicated that advanced age, lower education levels, a greater number of chronic diseases, and lower levels of family and social support were associated with higher care demands among the elderly in both urban and rural areas. These differences were statistically significant (P < 0.05). Conclusion: The elderly in urban and rural areas demonstrate a high demand for fall safety care. Particular attention should be given to individuals with lower education levels, those who are widowed, those with multiple chronic diseases, and those with low levels of family and social support to better meet the diverse care needs of this population. 展开更多

Objective:To evaluate the treatment effect of total hip arthroplasty(THA)for intertrochanteric femur fractures(IFF)in elderly patients.Methods:Thirty-two elderly patients with IFF admitted to the hospital from August 2021 to August 2024 were selected and randomly divided into two groups using a random number table.The experimental group(16 patients)underwent THA surgery,while the control group(16 patients)underwent proximal femoral nail antirotation(PFNA)surgery.Hip joint function and quality of life indicators were compared between the two groups.Results:Before surgery,there was no significant difference in hip joint function and quality of life scores between the two groups(P>0.05).However,at six months postoperatively,the experimental group had higher hip joint function and quality of life scores compared to the control group(P<0.05).The total effective rate was higher in the experimental group than in the control group(P<0.05).The complication rate in the experimental group was similar to that in the control group(P>0.05).Conclusion:THA can improve the clinical efficacy of elderly patients with IFF,minimize postoperative complications,effectively restore hip joint function,and optimize postoperative quality of life. 展开更多

Objective:To analyze the diagnostic value of combined testing of C-reactive protein(CRP)and procalcitonin(PCT)in elderly patients with bacterial pneumonia.Methods:This study included 50 elderly patients with bacterial pneumonia as the observation group and 50 patients with non-bacterial pneumonia as the control group,recruited from May 2022 to October 2023.Fasting venous blood samples were collected in the morning from all 100 participants.CRP levels were measured using a fully automated biochemical analyzer,while PCT levels were detected using the immunoturbidimetric luminescence method.Results:CRP and PCT levels were significantly higher in bacterial pneumonia patients[(98.25±11.59)mg/L and(3.57±1.35)μg/L,respectively]compared to the control group[(5.55±2.78)mg/L and(0.25±0.12)μg/L,respectively],with significant intergroup differences(P<0.05).Patients with severe bacterial pneumonia exhibited higher serum CRP and PCT levels compared to those with moderate or mild disease(P<0.05).The combined testing of CRP and PCT showed higher sensitivity and specificity than individual tests.In the observation group,CRP and PCT levels significantly decreased after treatment compared to pre-treatment levels.Conclusion:The combination of CRP and PCT testing provides high diagnostic accuracy for bacterial pneumonia in elderly patients.It effectively differentiates bacterial from non-bacterial infections,offering valuable data to guide clinical treatment. 展开更多

Background:To understand the current situation of loneliness and dementia fear among elderly people in the community;Analyze the influencing factors and clarify the correlation between the two.Methods:Through convenience sampling,228 elderly people were selected as the research subjects,and the general demographic characteristics questionnaire,elderly loneliness scale,dementia fear scale,and simplified intelligence status check scale were used for investigation.SPSS 28.0 statistical software was used for data analysis.Results:The total score of loneliness was 96.21±21.83 points,and the total score of dementia fear was 59.34±13.50 points.There were statistically significant differences in dementia fear scores among elderly people in the community in terms of age,gender,marital status,family history of dementia,and number of chronic diseases(P<0.05).Person correlation analysis showed a positive correlation between the total score of dementia fear and loneliness in elderly people in the community(r=0.162,P<0.05).Conclusion:The level of loneliness and dementia fear among elderly people in the community is at a moderate level.Age,gender,marital status,family history of dementia,and number of chronic diseases are important influencing factors of dementia fear among elderly people in the community.The sense of loneliness among elderly people in the community is positively correlated with dementia fear,meaning that the greater the degree of loneliness,the higher the degree of dementia fear among elderly people in the community. 展开更多

Correlation between diabetic peripheral neuropathy and thyroid hormone sensitivity in elderly patients with type 2 diabetes mellitus

BACKGROUND Diabetic peripheral neuropathy(DPN)is a common complication of type 2 diabetes mellitus(T2DM),significantly affecting patients’quality of life and imposing a substantial economic burden.Recent studies have highlighted the role of thyroid hormones in diabetes complications,particularly in elderly patients with T2DM.However,the relationship between thyroid hormone sensitivity and DPN remains unclear.AIM To investigate the correlation between thyroid hormone sensitivity and DPN in elderly patients with T2DM.METHODS In a cohort of 256 elderly patients with T2DM,propensity score matching was used to balance age,sex,and diabetes duration.Clinical data were collected to calculate thyroid hormone sensitivity and analyze its correlation with DPN.A random forest model was used to evaluate the diagnostic value of free triiodothyronine/free thyroxine(FT_(3)/FT_(4))for DPN.RESULTS Patients with DPN had a lower FT_(3)/FT_(4) ratio[(0.302±0.053)vs(0.316±0.049),P=0.040].Quartile stratification showed decreasing DPN prevalence with higher FT_(3)/FT_(4) ratios.Spearman’s correlation analysis showed that a lower FT_(3)/FT_(4) ratio was associated with higher glycated hemoglobin,fasting blood glucose,reduced nerve conduction velocity,and electrical skin conductance.Logistic regression indicated a positive relationship between the median FT_(3)/FT_(4) ratio and bilateral foot electrochemical skin conductance[odds ratio(OR):1.019;95%CI:1.005-1.034;P=0.007]and sural nerve sensory amplitude(OR:1.310;95%CI:1.008-1.703;P=0.043).Receiver operating characteristic analysis using a random forest model showed that incorporating FT_(3)/FT_(4) improved predictive performance for DPN,with an area under the curve of 0.74,sensitivity of 0.79,specificity of 0.64,and accuracy of 0.77.CONCLUSION In elderly patients with T2DM with euthyroidism,a lower FT_(3)/FT_(4) ratio is correlated with increased DPN incidence,affecting both large and small nerve fibers.FT_(3)/FT_(4) is an effective predictor of DPN. 展开更多
